(결과) 연령별 분포에서 30대와 40대가 각각 27% 와 26%로 높았으며, 남자(77.9%)가 여자(22.1%)보다 확연히 많았다. 손상의 원인으로는 교통사고가 72%로 가장 많았으며, 이 중 차량내 교통사고가 38.2%로 가장 많았다. 손상 장기로는 소장의 손상이 34.6%로 가장 많았으며, 다음으로 간(35%), 비장(19.1%), 장간막(12.5%)순이었다. 동반손상으로는 흉부손상(39%)이 가장 많았으며, 다음으로 두경부손상(31.6%), 골절손상(33.1%)이 많았다. 이환율은 40% 였으며, 이 중 창상감염(20%%)이 가장 많았으며, 특징적으로 대장, 직장의 손상시 창상감염(p=0.042)이 의의있게 높았다. 사망률은 18%(136명중 25명) 였으며, 사망의 원인으로는 저혈량성 쇼크(72%)와 패혈증(12%)이 많았으며, 사망률은 응급센터 도착당시 의식상태가 반혼수상태와 혼수상태인 경우(p<0.000), 수축기 혈압과 중간혈압이 60mmHg 이하인 경우(p=0.001), 혈색소가 8g/dl 이하인 경우(p<0.000), 혈소판수가 50,000/mm3 이하인 경우(p=0.032), creatinine이 1.6mg/dl 이상인 경우(p<0.000)에 의미있게 높았다. 손상 장기별 사망률은 장간막손상시 모두 생존하였으나 다른 장기 손상과 사망률과는 통계학적 의의가 없었다. (Background) Recently, abdominal trauma is more frequent following the increase of population, traffic accident, and violence. Especially, blunt abdominal trauma is a leading cause of morbidity and mortality among all age groups. Identification of serious intra-abdominal pathology is often challenging. Many injuries may not manifest during the initial assessment and treatment period. We analyzed the clinical features and the factors associated with morbidity and mortality. (Materials and Methods) We analyzed 136 patients of abdominal trauma who were admitted and treated in the Department of Surgery, Chonnam National University Hospital, from January 2003 to June 2005. We analyzed patients' age, sex, main residence, cause of trauma, injured organ, combined injury, mental status, blood pressure, laboratory findings, diagnostic method, morbidity mortality, and each variable by independent samples test and Kruskal-Wallis test. (Result) The age distribution ws predominant in 3rd(27%) and 4th(26%) decade. The sex distribution was 106 males(81.7%) and 30 females(22.1%) with ratio 3.5:1. Main residence distribution didn't show significant difference between urban and rural area. Among the 136 cases, 130 cases(95.5%) were blunt injuries and 6 cases(4.4%) were penetrating injuries. The causes of blunt trauma were traffic accident(98 cases, 72%), falling accident(9 cases, 6.6%), and violence(6 cases, 4.4%). The cause of penetrating trauma was stab injury(6 cases, 4.4%). Among the cases of traffic accident, in-car traffic accident was most frequent(52 cases, 38.2%). The injured organs were small intestine(47 cases, 34.6%), liver(35 cases, 25.7%), spleen(26 cases, 19.1%), mesentery(17 cases, 12.5%), large intestine(15 cases, 11.0%), pancreas(14 cases, 10.3%) etc, in order to decreasing frequency. The most commonly associated extraabdominal injury was chest injury(53 cases, 39%). Comatose or semicomatose mental status on admission were 7 cases with high mortality(85.7%). Shock status on admission(below 60mmHg in systolic) was related to high mortality(85.7%). In laboratory findings, decreased hemoglobin below 8g/dL, and platelet count below 50,000/mm3, increased creatinine level above 1.6mg/dL were significant prognostic factors. The most commonly used preoperative diagnostic modality was computed tomogram(93.4%). The incidence of postoperative complication was 40.4% and frequent complications were wound infection(8.1%) and rebleeding(8.1%). Overall mortality rate was 18.4%, and most common cause was hypovolemic shock(18 cases, 13.2%). (Conclusion) In multivariant analysis, mental status, hemoglobin, and serum creatinine level were most significant prognostic factors. To lower the morbidity and mortality, when abdominal trauma patient arrived at emergency room, rapid and accurate evaluation about patient's status and risk factors, and resuscitation have to be performed.
