It is mainly absorbed in the small intestine and excreted in the urine as unchanged form. The bioavailability of levosulpiride is only 30% after oral administration with large inter-individual variability. And, it was recently reported that levosulpiride serves as an in vitro substrate for PEPT1. The purposes of this study were to investigate the frequencies of the SNPs on PEPT1 in Korean population and to determine the effects of genetic polymorphisms in PEPT1 (G381A and G1287C) gene on pharmacokinetics of levosulpiride in healthy Korean subjects. Experimental subjects (627 and 636) were genotyped for G381A and G1287C of PEPT1 gene by pyrosequencing and restriction fragment length polymorphism (RFLP), respectively. The frequencies of 381A and 1287C were 42.34 and 6.29%, respectively. The pharmacokinetic profile of levosulpiride was examined in selected 192 subjects. A single dose of 25 ㎎ levosulpiride tablet was given orally to 192 healthy male Korean subjects. Serum and urine concentrations of levosulpiride were measured up to 36 hr using a validated HPLC method. In the exon 5 381AA and exon 16 1287CC genotype groups, the concentrations of levosulpiride were significantly lower than those of other groups (381GG and 1287GG) (P<0.05). The pharmacokinetic parameters of levosulpiride such as AUC0-4h, partial AUC of levosulpiride at absorption phase, AUC0-∞ and Cmax were significantly different among three genotypes on exon 5 G381A (P=0.029, P=0.010 and P=0.002). In exon 16 G1287C, statistically significant differences were observed among the genotype groups only for AUC0-4h (P=0.012). Also, in the 381A/1287C haplotype group, the concentrations of levosulpiride were significantly lower (P<0.05) than those in other group (GG/GG and heterozygous type), and AUC0-∞ and Cmax were significantly different among the haplotype groups (P=0.047 and P=0.036). The results indicate that levosulpiride is a substrate for PEPT1, and the pharmacokinetics of levosulpiride is clearly impaired in genotypes with PEPT1 SNPs. The partial amounts of levosulpiride excreted in urine were expected to be greater in 381AA and 1287GC genotype subjects, but observed to be greater in 381GG and 1287GG genotype (P=0.049 and P=0.002).
