PET/CT의 Standard uptake value (SUV)는 3.5이상의 섭취를 보일 때 악성병변으로 정의하였고, 전이병변은 2.5 이상의 섭취를 보이는 경우로 정의하였다. 15명 중 9명의 환자를 대상으로 악성간암의 발견여부를 평가하였고 간도관동맥화학색전술을 시행한 6명의 환자는 재발 혹은 남아있는 병변의 섭취를 관찰하였으며 15명의 환자 모두에서 전이여부를 검토하였다. 결과: 악성간암의 발견여부를 평가한 9명의 환자 중 7명에서 PET/CT상 13개 병변이 발견되었다 (최저 4.1, 최고 6.5). 이중 2개 병변은 초음파에서 보이지 않았으나 PET/CT에서 섭취증가를 보였다. PET/CT와 초음파에서 모두 보인 병변은 11개였고 초음파에서 보였으나 PET/CT에서 관찰되지 않은 병변은 2개(10cm, 11cm)였다. 전이여부를 평가한 15명의 환자중 5명에서 총 13개의 전이병변이 발견되었다 (최저 4.0, 최고 11.4, 평균 7.2). 경도관동맥화학색전술의 반응을 평가한 6명의 환자중 PET/CT상 2명의 환자가 섭취증가를 보였다 (SUV 10.4, 5.5). 나머지 4명의 환자는 섭취증가를 보이지 않았으며 이 중 초음파에서 병변이 보인 경우가 3명, 보이지 않은 경우가 1명이었다. 초음파에서 발견된 3명 중 1명의 병변은 SUV 0.8인 냉소로 나타나 재발성 병변이 아님을 알 수 있었으며 나머지 2명의 병변은 주변 간실질과 동일한 SUV를 보여 발견되지 않은 경우였다. Materials and Methods : From January, 2004, to September, 2005, 15 patients diagnosed with HCC were enrolled. The diagnosis of HCC was con-firmed by biopsy (n=7) and on the basis of ultrasound and CT evaluation (n=8). Of these 15 patients, TACE was done in six patients. SUV of 3.5 of more was used as suggestive of primary hepatic tumor and 2.5 or more as metastatic hepatic tumor. PET/CT was analyzed to evaluate primary lesion, metastatic lymphadenopathy and distant metastasis (n=9), and also assessed treatment response following TACE (n=6). Results Thirteen primary lesions are found in seven patients. Two lesions of these were not seen in ultrasound. Eleven lesions were found with PET/CT and ultrasound. Thirteen metastatic lesions were found in five paitents. Of six patients who underwent TACE, two patients showed increase in SUV. Four patients showed normal SUV uptake. Conclusions : In staging of HCC and assessment of TACE, combined in-formation from the ^(18)F-FDG PET/CT and unenhanced CT portions of a PET/CT are useful.
