Background: As Parkinson’s disease (PD) is a chronic progressive neurodegenerative disorder, the treatment for the promotion of quality of life (QOL) has also been taken into consideration. The purpose of this study is to assess the interrelation between pain and PD and to evaluate the impact of pain on the QOL of PD patients and the effect of PD on pain. Methods: Eighty-two patients were included. To find the causal relationship between PD and pain, PD patients were classified into the PD with pain group and PD without pain group. The PD with pain group was subdivided into PD-related pain groups and PD-unrelated pain groups. To evaluate the QOL, Medical Outcomes Study 36-Item Short Form Health Survey (SF-36) was used. Parkinsonian disability was assessed by detailed examination and review of medical records using Hoehn and Yahr scale and United Parkinson’s Disease Rating Scale (UPDRS). Modified Somatic Perception Questionnaire (MSPQ), Zung depression inventory-Self rating Depression Scale (SDS), Visual Analogue Scale (VAS) and pain specific questions were performed by one on one interviews. The causal relationship between pain and PD was investigated by non-parametric analysis and multiple stepwise regression analysis. Result: UPDRS part III scores, VAS scores, SF-36 scores, SDS scores, MSPQ scores, and gender illustrated a significant difference between the PD with pain group and the PD without pain group. Among the pain group, there was no difference between the PD-related pain group and the PD-unrelated pain group. Pain had the most detrimental impact on the QOL of the PD patients, estimated by the physical component summary of SF 36. Conversely, parkinsonian symptoms and signs evaluated by UPDRS motor sum scores also had an influence on pain. Conclusion: Parkinsonian motor symptoms have provoked pain. Both the PD-related pain and PD-unrelated pain could deteriorate the QOL of PD patients. Therefore, the pain in PD must be considered in the management of PD. However, once the pain has developed in the PD patient, whether the pain is related to the PD or not is not statistically significant.
