The present study purposed to examine how foot orthoses and lumbar stabilization exercise biomechanically affect foot malalignment, pelvic displacement and lumbar displacement observed in spine malalignment patients with flexible flatfoot. The subjects of this study were 20 male and female spine malalignment patients with flexible flatfoot, and their malalignment before and after experiment was examined using weight bearing X-ray. Of the subjects, the non exercise group (n=10) wore foot orthoses for 8 weeks, and the exercise group (n=10) wore foot orthoses for 8 weeks and had lumbar stabilization exercise 3 times a week for 8 weeks in the 1st and 2nd stages. With collected data, we carried out paired t-test to examine differences in non exercise group between before and after the experiment and in the exercise group between before and after the experiment, and one way ANOVA through repeated measuring to examine differences between the non exercise group and the exercise group. Significance was determined at p<.O5, and the following results were obtained. 1) According to the result of analyzing difference in foot malalignment angle before and after wearing foot orthoses, the talo-first metatarsal bone angle increased significantly in both feet of the non exercise group and the exercise group (p<.005). The hallux valgus angle decreased significantly in the left foot of the non exercise group and the exercise group and the right foot of the exercise group (p<.001) but no significant difference was observed in the right foot of the non exercise group (p>.O5). The angle between the 1st~2nd metatarsal bones did not show any significant difference in both feet of the non exercise group and the exercise group (p>.05). 2) According to the result of analyzing difference in pelvis malalignment angle before and after wearing foot orthoses, the obliquinty angle of the head of femur decreased significantly in both the non exercise group and the exercise group (p<.05). The inclination angle of sacrurn decreased significantly in both the non exercise group and the exercise group (p<.005). 3) According to the result of analyzing difference in lumbar instability angle before and after wearing foot orthoses, the lumbosacral disc angle increased significantly in the exercise group (p<.05) but did not show any significant difference in the non exercise group (p>.05). Lumbar lordotic angle decreased significantly in both the non exercise group and the exercise group (p<.001). 4) As to foot malalignment angle after wearing foot orthoses, the tab-first metatarsal bone angle showed a significant difference between the non exercise group and the exercise group in both feet (p<.05). This study examined only the relation between foot malalignment and spine malalignment, but further research is considered necessary to examine the biomechanical interrelation among the feet, lower limbs, spine and skull.
