H. pylori 균주의 내성 판정 기준은 최소억제농도가 metronidazole 8 μg/mL, clarithromycin 1 μg/mL, amoxicillin 1 μg/mL 이상으로 하였다. 일차치료로 PPI, clarithromycin, amoxicillin 3제 병합요법을 7일간 사용하였고 치료가 종료된 이후 4-6주후에 내시경검사를 시행하거나 요소호기검사로 제균 판정하였다. 일차 치료에 실패한 환자들에 대해서 PPI, bismuth, tetracycline, metronidazole 4제 요법을 14일간 투약하였다.
결과: 466명의 소화성 궤양환자에서 급속 요소분해효소검사 또는 조직검사로 H. pylori 감염이 확인된 환자는 265명이었고, 그중 144명의 환자에서 H. pylori가 분리되어 배양률은 54.3%이었다. The aim of this study was to assess the changing pattern of antibiotic resistance of H. pylori strains in peptic ulcer patients and to evaluate the eradication rate of antibiotic resistant H. pylori strains. Methods. One hundreds forty four H. pylori isolates obtained from 466 patients with peptic ulcer disease between June 2001 and December 2005 were tested for antimicrobial resistance. The minimum inhibitory concentrations (MIC) for metronidazole, clarithromycin and amoxicillin were determined by the modified broth micro-dilution, E test method, and disc diffusion test. Isolates were considered resistant when the MIC was more than 8μg/ml for metronidazole, more than 1μg/ml for clarithromycin and more than 1μg/ml for amoxicillin. Results. Overall, resistance rates to metronidazole and clarithromycin were 41.9% and 5.4% of isolates, between June 1996 and March 2000. Resistance to metronidazole and clarithromycin was detected in 48.6% and 16.7% of isolates, respectively between January 2001 and December 2005. During the recent 5-year study period, amoxicillin-resistant H. pylori strains were 11.8%, and multi-drug resistance rate of H. pylori was 16.7%. Seven (4.8%) strains were resistant to all the three antibiotics. The eradication rates of clarithromycin containing triple therapies is low (7.8%) for clarithromycin-resistant strains. Conclusions. There was no significant increase in the prevalence of metronidazole resistance, but clarithromycin-resistant H. pylori strains have increased significantly over the 10-year period. There was an increasing tendency for the emergence of strains with multi-drug resistance. The increase in resistance to clarithromycin results in a decrease in eradication efficacy for H. pylori. These results indicate that the antibiotics used for H. pylori eradication show high resistance. Continuous surveillance of antibiotic susceptibilities should be needed and further increases in antibiotic resistance would require susceptibility testing before treatment to maximize the efficacy of H. pylori treatment. In areas with high clarithromycin resistance, new alternative first line treatment strategies should be considered.
