인간의 삶에 있어 중요한 것 중의 하나는 건강이라 생각한다. 그러한 삶 의 과정 중에 암이라는 진단을 받았을 때 죽음과 고통을 연상하게 될 것 이다. 이러한 가운데 현대의학의 암에 대한 치료의 한계에 봉착하게 될 때, 암 치료에 도움이 된다면, 그 무엇이든 해보고 싶은 것이 대부분의 많은 사람들의 심리일 것이다. 이러한 현실에서 암을 진단 받은 환자들은 전에도, 지금도 양성적, 음성적으로 보완대체요법들을 많이 사용하고 있다고 본다. 따라서 본 연구는 보완대체요법의 사용실태를 파악하여 암 환자에게 보다 정확한 정보를 제공하는 기초 자료로서 도움이 되고자 하는 뜻에서 이 연구를 시작하게 되었다. 본 연구는 서울 일개 모 W 병원에 방문한, 암 진단 1개월이 지난 환자들을 연구대상으로 하여 보완대체요법 사용실태, 효과, 부작용 등을 조사하기 위한 서술적 연구이다. 입원 및 외래를 통해 치료를 받고 있는 암환자 총 291명을 대상으로 설문지를 이용하여 2007년 3월 5일부터 3월 25일까지 20일간 조사하였다. 수집된 자료는 SPSS 11.5 for Windows 프로그램을 이용하여 분석하였으며, 자료의 분석을 위하여 빈도분포와 백분율을 구하였고, 일반적 특성에 따른 보완대체요법 분석은 보완요법 종류별 사용을 복수 응답으로 조사하여 빈도 분포와 백분율로 분석하였으며, 보완대체요법의 시작 시기, 사용 기간 및 사용 금액은 평균치 분석을 실시하였다. 이외에 보완대체요법 사용과 관련된 항목들에 대해서도 빈도 분포 및 백분율로 분석한 결과 다음과 같은 결론을 얻었다. 1. 연구 대상자의 인구 사회학적 특성은 여자가 57.7%. 남자가 42.3%로 여자가 많았으며, 연령은 50~59세 사이가 가장 많았고, 교육정도는 고졸이 44.0%로 가장 많았다. 종교는 무교가35.4%로 가장 많으나, 종교를 가지고 있는 그룹에서는 기독교(29.2%), 불교(24.1%) 순으로 결과 되었다. 미혼보다 기혼이 91.4%로 많았고, 자녀를 가지고 있는 사람이 83%로 많았으며, 동거형태는 배우자하고만 거주가 66.6%, 가계 월 총수입은 100~200만원 사이가 29.2%로 가장 많았다. 2. 연구 대상자의 질환관련 특성은 대장암(19.6%), 유방암(16.2%), 인두,폐 암(14.8%), 식도,위암(10.4%), 간담도계통암(9.3%), 난소,자궁암(9.3%)골육종(8.9%), 림프,혈액암(5.8%), 기타(5.5%) 순으로 나타났고 질병 이환기간은 1~6개월 사이가 46.1%로 가장 많았다. 치료내역에 있어서는 항암요법만 받은 상태가 43.6%로 가장 많았고, 다음으로 수술과 항암요법의 복합요법이 29.6%로 나타났으며, 간병을 해주는 대상자 질문에는 배우자의 응답이 55.3%로 가장 많이 나타났다. 3. 연구대상자의 75.6%에서 보완대체요법 사용 경험이 있었으며 고졸에서 44.4%로 가장 많이 사용하였고, 종교가 있는 사람, 기혼, 자녀가 있는 사람이 보완대체요법 사용 경험이 많았으며 가계 월 총수입이 100~200 만원 사이의 사람이 이용경험이 많았다. 4. 연구대상자의 질환특성에 따른 보완대체요법 사용은 암의 종류에 있어통계적으로 유의성을 보이지 않았으며, 골육종(84.6%), 간담도계통암 (81.5%), 인두,폐암(79.1%), 대장암(79.0%), 식도,위암(77.4%), 유방암(72.3%)순으로 나타났다. 암의 이환기간의 차이는 37개월 이상이 92.7%로 가장 많은 사용을 보인 반면 1~6개월 사이는 67.7%로 질병의 이환기간이 길수록 보완대체요법 사용의 증가를 보이는 통계적인 유의한 차이를 나타냈다. 5. 연구대상자 75.6%의 보완대체요법 사용자는 건강식품(63.9%)로 가장 많이 사용하였으며 다음으로 식이요법(58.1%), 양방요법(44.0%), 심신조정 요법(30.6%), 수기신체요법(24.7%), 물요법(21.7%), 한방요법(18.6%), 기타(4.5%)로 나타났다. 6. 연구대상자의 보완대체요법 사용의 시작시기와 기간은 매우 다양하나 평균 암 진단 후 시작 시기는 2.5±4.6개월이고 평균기간은 8.7±17개월로 나타났다. 또한 보완대체요법 사용총비용은 평균 271.3±758.0만원으로 나타났으며 그에 대한 사용비용에 대해 부담이 크게 느끼고 보완대체요법 사용비용은 가족이 부담하는 것으로 52.7%로 가장 많았다. 7. 보완대체요법 사용에 대한 정보는 27.3%가 신문이나 방송을 통해 얻고 있는 것이 제일 많았고 다음으로 가족(21.5%), 경험자(19.0%), 친지 (10.2%), 이웃(7.8%) 순으로 나타났다. 8. 보완대체요법의 사용 동기는 다양한 응답이 있었으나, 42.3%가 병의 치 료에 도움이 될 것을 기대한 응답이 가장 많았고, 다음으로 병의 완치(20.6%), 병원치료만 하기엔 불안해서(14.4%), 심리적 안정(11.0%), 남들이 사용해서(2.8%) 순으로 나타났다. 9. 보완대체요법 사용 한 후의 효과에 대해서는 “잘 모르겠다”와 “심리적으로 편안하였다“가 각각 38.1%로 가장 많아 나타났으나, 질병호전은 4.8%로 유의한 차이를 보이고 있다. 또한 사용 후 의 부작용에 대해서는 ”없었다“ 가 93.9%로 응답 하였으나, 6.1%에서는 설사, 간수치 상승, 소화불량, 가려움증, 부종, 명현반응 등의 부작용을 경험한 것으로 나타났다. 10. 의료진에게 보완대체요법 사용고지를 하지 않는 경우가 55.7%로 가장 많이 나타났으며, 고지를 하는 경우에는 의사(11.3%), 간호사(6.9), 약사(0.7%)의 순으로 나타났다. 또한 의료진에게 고지를 하지 않은 이 유에 있어서는 상의 불필요성이 36.7%로 가장 많았고 다음으로 의료 진이 싫어할까봐(29.9%), 상의할 기회가 없어서(29.4%), 기타(4%)순으 로 나타났다. 11. 291명의 연구대상자 중 24.4%는 보완대체요법 사용하지 않았다. 그 이유에 대해서는 32.8%는 보완대체요법이 과학적으로 증명되지 않아서, 효과가 의심이 되어 사용하지 않는다고 응답하였으며, 부작용 발생의 우려가 28.4%, 의료진의 반대가 23.9%, 본인이 싫어서가 9.0%, 비용이 부담이 되어서가 6.0%로 나타났다. 결론적으로 이상의 연구결과를 종합하여 보면, 많은 암 환자들은 병원치료를 받고 있으면서도 보완대체요법을 다양하게 사용하고 있으며, 병의 이환기간이 길어질수록 더 많이 사용하고 있었다. 보완대체요법의 사용으로 인해 심리적으로 안정감과 편안함을 느끼고 있으나, 효과의 결과는 “잘 모르겠다”는 응답이 많은 것으로 보아 보완대체요법의 안정성과 과학적인 검증을 필요로 하다고 볼 수 있다. 이런 것들을 고려하여 볼 때 의료인의 보완대체요법에 대한 이해와 관심을 통해 바람직한 사용을 격려하고 보완대체요법의 무분별한 사용으로 인한 비용손실 및 부작용을 예방할 수 있는 체계적인 홍보와 교육적 관심이 요구된다.

So confronted with the limitations of cancer treatment by modern medicine, many people would be willing to try anything helpful in curing cancer. Under the circumstances, it is believed that patients diagnosed with cancer used, and are using, many complementary and alternative therapies both legally and illegally. The purpose of this study is to examine the actual use of complementary and alternative therapies and serve as basic data to provide more accurate information to cancer patients. This is a descriptive research on the actual use, efficacy, and side effects of complementary and alternative therapies in patients who visited W Hospital in Seoul and who had received a diagnosis of cancer for more than one month. A survey was conducted with a total of 291 patients receiving treatment as inpatients or outpatients for 20 days from March 5 to 25, 2007. The data were analyzed by using an SPSS 11.5 for Windows program; frequency distribution and percentage were obtained for data analysis; for analysis of complementary and alternative therapies by general properties, the use of complementary therapies was investigated with multiple responses by kinds for frequency distribution and percentage; and mean analysis was carried out for the time of starting a complementary or alternative therapy, the period of using it, and costs of using it. Besides, another analysis was performed on items related to the use of complementary and alternative therapies for frequency distribution and percentage, consequently drawing the following conclusions. 1. As for demographics of subjects, there were more females (57.7%) than males (42.3%); the largest portion of them were between 50 and 59 of age; 44.0% were high school graduates. As for religion, 35.4% had no religion; among religious groups, 29.2% were Christians and 24.1% were Buddhists. 91.4% were married; 83% had children; 66.6% were living with a spouse alone; and 29.2% earned the total monthly household income of one to two million won. 2. As for subjects' disease-related properties, large intestine cancer (19.6%) made up the largest portion, followed by breast cancer (16.2%), pharynx/lung cancer (14.8), esophageal/stomach cancer (10.4%), liver/biliary tract cancer (9.3%), ovarian/uterine cancer (9.3%), osteosarcoma (8.9%), lymphoma/hematologic malignancy (5.8%), and others (5.5%), and 46.1% had got the disease for one to six months. As for treatment, 43.6% were getting anti-cancer treatment alone, followed by a combination of surgery and anti-cancer treatment, and 55.3% were having their spouse nurse them. 3. 75.6% of subjects had experience of using a complementary or alternative therapy, 44.4% of whom were high school graduates, and those who had religion, who were married, who had children, and who earned the total monthly household income of one to two million won had much experience of using it. 4. There was no statistical significance in the use of complementary and alternative therapies by kinds of cancer: osteosarcoma got the highest rate (84.6%), followed by liver/biliary tract cancer (81.5%), pharynx/lung cancer (79.1%), large intestine cancer (79.0%), esophageal/stomach cancer (77.4%), and breast cancer (72.3%). As for morbidity of cancer, 92.7% of those who had got it for more than 37 months used a complementary or alternative therapy while 67.7% of those who had got it for one to six months used it; thus, the longer the period of morbidity, the more the use of a complementary or alternative therapy, showing statistically significant differences. 5. Of 75.6% of the subjects using a complementary or alternative therapy, 63.9% were using health foods, followed by a dietetic treatment (58.1%), Western therapy (44.0), mind/body therapy (30.6%), manipulative and body-based method (24.7%), water therapy (21.7%), Oriental therapy (18.6%), and others (4.5%). 6. While the time of starting a complementary or alternative therapy or alternative therapy and the period of using it by subjects varied significantly, they started it 2.5±4.6 months after the diagnosis of cancer and continued it for 8.7±17 months on the average. The total costs of using a complementary or alternative therapy amounted to 2.713±7.58 million won on the average and 52.7% suffered from the burden of the costs and had their family bear the burden. 7. As for the sources of information on using complementary and alternative therapies, 27.3% got the information via newspaper or broadcasting, followed by family (21.5%), experienced persons (19.0%), relatives (10.2%), and neighbors (7.8%). 8. While there were various motivation to use complementary and alternative therapies, 42.3% suggested help in curing the disease, followed by perfect cure of the disease (20.6%), anxiety about hospital treatment alone (14.4%), psychological stability (11.0%), and use by others (2.8%). 9. As for efficacy of complementary and alternative therapies, each of "little confidence" and "psychological comfort" got 38.1% while improvement in conditions got 4.8%, showing significant differences. 93.9% suffered from "no side effect" from using them while 6.1% suffered from such side effects as diarrhea, the increase in hepatic rate, indigestion, pruritus, edema, and dizziness. 10. 55.7% didn't notify medical staff of the use of complementary and alternative therapies; notification was made to doctors (11.3%), followed by nurses (6.9%) and pharmaceutists (0.7%). 36.7% didn't make no notification to medical staff because of no need of discussion, followed by dislike by medical staff (29.9%), no opportunity to discuss (29.4%), and others (4%). 11. Of 291 subjects, 24.4% used no complementary or alternative therapy. 32.8% failed to use it because it was never proved scientific and doubtful of efficacy, followed by possible occurrence of side effects (28.4%), objection by medical staff (23.9%), dislike by themselves (9.0%), and the burden of using it (6.0%). To put the results of this study, many cancer patients were using various complementary and alternative therapies while receiving hospital treatment; the longer morbidity, the more they used such therapies. While the use of complementary and alternative therapies made them psychologically stable and comfortable, many subjects "failed to be confident" of their efficacy; therefore, it is necessary to make scientific test of complementary and alternative therapies for stability. Considering these results, medical staff should encourage patients to make desirable use of complementary and alternative therapies based on understandings and concerns about them and provide systematic publicity and educational concerns to prevent financial loss and side effects from using them indiscreetly.

As for treatment, 43.6% were getting anti-cancer treatment alone, followed by a combination of surgery and anti-cancer treatment, and 55.3% were having their spouse nurse them. 3. 75.6% of subjects had experience of using a complementary or alternative therapy, 44.4% of whom were high school graduates, and those who had religion, who were married, who had children, and who earned the total monthly household income of one to two million won had much experience of using it. 4. Considering these results, medical staff should encourage patients to make desirable use of complementary and alternative therapies based on understandings and concerns about them and provide systematic publicity and educational concerns to prevent financial loss and side effects from using them indiscreetly.
