Relationship between stroke and diurnal rhythm of cortisol secretion has been studied profoundly. Most of them are limited to the acute state of stroke. Cortisol secretion in those psychological symptoms without stroke has been studied consistently and it shows different patterns of cortisol secretion from those of normal people. Stroke patients after the acute stage develop psychological symptoms: depression, instability, insomnia, fatigue, etc. Therefore, we investigated the diurnal rhythm of cortisol secretion in stroke patients after the acute stage. Subjects: Cerebral infarction and cerebral hemorrhage patients (at least 5 days after onset) without disturbance of mentality got participated. Normal group was age-controlled matched. Methods: Salivary cortisol levels at post-awakening and before bed were counted in stroke group and normal group. Results: 1. Cortisol level of Stroke group showed a normal distribution after being log transformed and there was no difference between male and female. 2. While cortisol level of stroke group at post-awakening is slightly lower than that of control group, cortisol level of stroke group at before bed is significantly higher than that of normal group. 3. Although cortisol level of cerebral hemorrhage group at post-awakening is slightly lower than that of control group, cortisol level of stroke group at before bed is significantly higher than that of normal group. 4. Although cortisol level of cerebral infarction group at post-awakening is slightly lower than that of control group, cortisol level of stroke group at before bed is significantly higher than that of normal group. 5. There was no difference of cortisol levels at both post-awakening and before bed between cerebral hemorrhage group and cerebral infarction group. Conclusion: Cortisol circadian rhythm of stroke patients after the acute stage is flattened. This may cause the various psychological symptoms. Further research is needed about the relationship between brain damage and the instability of cortisol circadian rhythm of stroke patients after the acute stage.
