The aim of the study was to investigate the effects and mechanism of L. sakei on moderate or severe atopic dermatitis (AD) in children. METHODS: Sixty six children with moderate or severe AD were recruited into a randomized double-blind placebo- controlled trial; 54 children completed the study. The children were given a probiotics (Lactobacillus sakei, NFM proBio65 1x1010/day) or an equivalent volume of placebo for 12 weeks. As object parameter of improvement and methods for knowing mechanism of action, serum cytokines and chemokines (IL-4, IL-13, IFN-γ, IL-10, TGF-β1, β-depensin, IL-31, CCL17/TARC, CCL27/CTACK, CCL18/PARC) were measured. RESULTS: The main outcome measures were severity and extent of AD at the end of the study, as measured by the Severity Scoring of Atopic Dermatitis (SCORAD) index. The significant reduction in the SCORAD index over time was 8(29.6%, n=27) in the control group, 17(70.0%, n=27) in the probiotics group. (P<0.05) Serum CCL17/TARC, CCL27/CTACK, CCL18/PARC level was correlated with SCORAD index (P<0.05, P<0.001, P<0.001) and was significantly decreased after treatment. (P<0.01, P<0.01, P<0.05) There was significant decrement of serum TGF-β level in probiotics group. (P<0.05) CONCLUSION: Supplementation with L. sakei is beneficial in improving the severity of children with moderate to severe atopic dermatitis. Decreased serum level of CCL17/TARC, CCL27/CTACK, CCL18/PARC is objective evidence of improvement.
