The purpose of this study was to identify the postoperative effects of the high and low resistant elastic band exercise on shoulder joint dysfunction and lymphedema of the breast cancer patients. For seventeen woman patients suffering from shoulder joint dysfunction and lymphedema subsequently developed after the unilateral breast cancer operation were randomly allocated to high(n=9) or low(n=8) resistance exercise group. Both groups were participated in the elastic band exercise program three times a week lasted for eight weeks. For the high resistance exercise group(HG), the resistance was gradually increased, while maintaining constant intensity of exercise for the low resistance exercise group(LG). Assessments made include the range of shoulder motion(inclinometer, Dualar IQ), the upper extremity muscle strength(Nicholas MMT), the pain(visual analogue scale), and lymphedema(by using a measuring-tape for the circumference measurement). Paired t-tests were made to examine the statistical significance of changes in each group pre-test and post-test, and indepentant t-tests were made to compare the two groups. The results of this study were as follows. 1. After the exercise program, the shoulder horizontal abduction was significantly increased in both of HG and LG(p < .05), but the difference between the two groups was not significant. 2. After the exercise program, the improvement of upper extremity strength was significantly higher in the HG than in the LG (p <.05). 3. After the exercise program, pain score was not significantly changed in both of HG and LG. 4. After the exercise program, upper arm edema rate was significantly decreased in HG(p < .05), but the difference between the two groups was not significant. In conclusion, this study suggest that the elastic band exercise helps improve shoulder joint dysfunction of the breast cancer treated women. Especially, high resistance exercise is more effective in improving muscle strength and does not exacerbate lymphedema, rather may improve upper arm edema if it is applied with low elastic bandage or compression sleeve.
