The study was designed as a quasi-experimental, nonequivalent control group pre-post test design. The subjects were over 60-years-old elders belonged to churches in Gyung gi province and they gave written informed consent. Thirty-two people who suffered from insomnia were conveniently assigned to an experimental group(n=16) or a control group(n=16). The data were collected from March to September 2007. The experimental group went through four weeks (group training once a week and home training everyday) of sleep management training, which was followed by feedback teaching for each experiment. Every single subject was provided with hygienic sleep teaching material and progressive muscle relaxation tapes so that they could take advantage of them 30 minutes before sleep. Their experimental performance was encouraged by phone calling and they were told to hand in performance cards after answering yes or no. Each week, subject performance cards were gathered, and the researcher gave feedback. For both two groups; general characteristics, sleep scores, sleep satisfaction scores and immune responses were respectively measured before and after the experiment. A sleep measurement tool, developed by Jin-ju Oh(1998), was used to survey the sleep score. The sleep satisfaction was evaluated by visual analogue scale(zero to ten). T3(CD3), T4(CD4), and B cell(CD19) were examined to find out immune response and the data were analyzed by the chi-square test, Fisher's exact test, t-test and ANCOVA using the SPSS/WIN program. The results were as follows: 1. The experimental group showed higher sleep scores than those of the control group(t=7.86, P=.00). 2. The experimental group showed higher sleep satisfaction scores than those of the control group(t=7.61, P=.00). 3. B cell in immune response was increased in the experimental group(F=23.71, P=.00).
The results of this study indicate the sleep management training can increase the sleep score, sleep-satisfaction score and partial immune response in elderly people suffering from insomnia. From these findings, we can expect that sleep quality and satisfaction can be improved by applying sleep management training program to the elderly complaining of insomnia as a unique nursing implementation.
