A flatfoot is defined as a progressive or acquired deformity of foot. It has structural changes such that longitudinal arch is lower, talus rotates inside toward sole, medial longitudinal arch is lower than before, and the forefoot has deformity of supination and eversion. Studies based on the influence of body by impulse growth from loads or dynamic activities such as landing from higher than ground are limited for flatfoot. Thus, this study quantified diverse variations of kinematic and kinetic factors when subjects are landing and walking with external loads in flatfoot and normal foot. This study also investigated the prevention from confirming risk factors of injury and basic data of exercise during daily activities in flatfoot. Twenty-three subjects with flatfoot and twenty-two adults with normal foot were recruited along with their written informed consent. They walked on a ten-meter walkway at a self-selected pace with loads of 0, 5, 10, and 15㎏ and performed a drop-landing task at the height of 20, 40, and 60㎝. They completed three trials in each condition and biomechanical changes were measured. Kinematic and kinetic changes were measured by Vicon system and force flate, measured by MP150 system, and foot pressure was measured by RS-scan system. Joint angle in sagittal plane, muscle activity, and vertical ground reaction force were increased in each group in accordance with heights during landing. During landing, all of the above were increased more in flatfoot compared with normal foot. Classifying displacement as joint angle and muscle activity in lower extremity during walking with loads was challenging. Foot pressure was concentrated in metatarsal bones. These findings revealed that flatfoot increases risk factors of body damage because of biomechanical mechanism since impulse absorption of flatfoot is decreased in daily activities. Accurate clinical assessment for risk factors of body damage in flatfoot was complex. Therefore, future studies should evaluate flatfoot in various methods and focus on prevention from damage due to external conditions. Also more research should be considered for treatment exercises and orthosis to treat flatfoot along with accurate assessment.
