Characteristics of respiratory functions and phonation in patients with young-onset parkinson's diseases or normal adult원문보기

호흡기능 평가는 Cosmed의 MicroQuark^(Ⓡ)로 FVC, FEV1과 FEV1/FVC을 측정하여 폐의 환기기능을 위한 기초평가를 하였고, 음성표집은 KayPENTAX의 Computerized Speech Lab(CSL) Model 4400의 Multi-Dimentional Voice Profile(MDVP) Advanced Model 5105로 Maximum Phonation Time(MPT), jitter와 shimmer를 분석하였고, KayPENTAX의 Phonatory Aerodynamic System(PAS) 4500으로 Mean airFlow Rate(MFR)과 Subglottal Pressure(Psub)를 분석하였다.

Parkinson’s disease is the most common degenerative disease of neuropathy, and the average onset age of the whole patients’ group is from 50 to 69.
However, ‘young-onset parkinson's disease (YOPD)’ can occur even before 49.
The speech characteristic of parkinson’s disease patients is hypokinetic dysarthria, and they have little tidal air when they phonation, show unstable vocal patterns, have weakened muscles related to larynx, and thereby indicate monotone, monoloudness, breathiness, and reduced loudness, and roughness in general. These characteristics can be resulted from problems in respiration function or degenerative changes in larynx regulation or laryngeal tissue. Therefore, this study selected YOPD patients to exclude the confounding variable of ‘age’ and examine the vocal characteristics of parkinson’s disease purely, and it aims to investigate their respiration function and phonation characteristics comparatively with those of the normal group.
This study has as its subjects five YOPD patients’ group whose chronological ages are between 38 and 49 and onset ages are under 49 and also five normal people’s group selected by frequency-matching. For testing their respiration function, FVC, FEV1 and FEV1/FVC were measured with Cosmed’s MicroQuark^(Ⓡ) as a basic test for the ventilation function of the lung. For sampling vocal, Multi-Dimensional Voice Profile (MDVP) Advanced Model 5105 of KayPENTAX’s Computerized Speech Lab (CSL) Model 4400 were used to analyze Maximum Phonation Time (MPT), jitter, and shimmer. And KayPENTAX’s Phonatory Aerodynamic System (PAS) 4500 was utilized to analyze Mean airFlow Rate (MFR) and Subglottal Pressure (Psub). Otolaryngologists and neurologists participated in the research cooperatively, and Otolaryngologists tested whether the larynx had disease or not directly through larynx stroboscopy in normal adult.
Mann-Whitney’s U test was used to analyze the difference between the two groups’ respiration and phonation function, and Pearson’s correlation analysis was conducted to examine the correction between the two groups’ respiration function and phonation.
As a result, MPT was statistically significantly longer in the normal group than YOPD patients’ group, and both jitter and shimmer was statistically significantly larger in YOPD patients’ group than the normal group. However, there wasn't statistically significant difference in FVC, FEV1, FEV1/FVC, MFR, and Psub. According to the examination of correlation between YOPD patient's group and the normal group’s respiration and phonation function, there was no correlation between the variables. The two groups’ respiration function does not differ, and YOPD’s MPT, jitter, and shimmer differ beyond the normal range, and it is thought to be resulted from problems in airflow regulation of the larynx or degenerative changes in larynx tissue rather than problems in YOPD patients’ ventilation function of their lung when they phonation. Thus, these variables will help perform YOPD’s phonation diagnosis and therapeutic plans or anticipate recuperation. Although there have been studies on the phonation of parkinson’s disease patients, we can hardly find advanced researches on the respiration and phonation function of YOPD patients’ vocal characteristics. Therefore, this article is meaningful in that it excludes the confounding variable of ‘age’ and understands Parkinson’s disease.
