Clear speech는 파킨슨병 환자들의 말명료도를 최대한 증가시키기 위한 테크닉으로 소개되고 있다. Clear speech는 본질적으로 증가된 음성강도와 말속도를 감소시키는 치료기술의 결합이다. 본 연구에서는 Clear speech가 파킨슨병 환자들에게 적절한 중재방법이 될 수 있는지 확인하고 LSVT와 간접적인 비교를 실시하고 어떤 차이를 보이는지, 그 의미가 무엇인지 확인하고자 하였다. 연구방법: 본 연구에서는 파킨슨병 환자 15명을 대상으로 주 2회, 4주 동안 총 8회기의 Clear speech 중재를 실시하고, 중재 전, 후의 음향학적/공기역학적, 청지각적, 자가-지각적 변화를 통해 말명료도 향상에 어떤 변화를 보이는지 알아보고, Clear speech 중재 프로그램이 파킨슨병 환자들의 임상적인 전략으로 사용될 수 있을지 확인하고자 하였다. 총 20명을 모집하였으나, 5명은 중도 탈락으로 연구에 참여한 대상자는 15명(M=7, F=8)이다. 대상자들은 모두 도파민 약을 복용하고 있고, 언어치료를 받아 본적이 없거나, 받은 지 2년이 지난 대상자들이며, Hoehn & Yahr stage 점수는 3점 미만이다. Clear speech 중재 프로그램의 치료시간은 약 45-50분이며 각 회기는 연구자와 파킨슨병 환자가 1대 1로 만나 개별적으로 이루어졌으며 개별 회기가 끝난 후 다음 회기까지 과제를 제공하여 환자가 집에서도 목표 음성을 위한 연습을 실시하도록 하였다. 중재 전 기초선 측정을 실시하고 4주 간의 중재 프로그램 이후 변화를 측정하였다. Clear speech 중재 프로그램 전, 후의 변화는 음향학적, 공기역학적 평가, 청지각적 평가, 자가-지각적 평가를 통해 실시하였으며 모두 평균과 표준편차로 계산되었다. 통계적 검증은 정규성 검정을 실시한 후, 대응 표본 t 검정과 윌콕슨 부호 순위 검증으로 분석하였다. 추가적으로 청지각적 평가에서 말명료도에 영향을 줄 수 있는 요인들과의 상관관계를 확인하기 위해 피어슨 상관 분석도 실시하였다. 중재 전, 후 데이터는 95% 신뢰수준 (p<.05)을 기준으로 분석하였다. 연구결과: Clear speech 중재 프로그램 전, 후의 음향학적, 공기역학적 변화를 확인해 본 결과, 습관적 피치가 28.06Hz 증가하였고, 피치 범위가 42.24Hz 증가였다. 말속도는 1.65SPS 느려졌으며, 호흡 수가 4.8회 증가하였다. 강도와 평균 호기류율은 유의미한 결과는 보이지 않았지만, 파킨슨병 환자들의 말명료도 증가를 위해서 음성크기가 특별하게 증가해야 하는 것은 아니며, 특히 Clear speech는 과장된 발화 혹은 과조음에 초점을 맞춘 중재 프로그램이기 때문에 음성강도가 증가되지 않더라도 부정확한 조음과 운율 문제를 해결하여 말명료도 증가에 기여할 수 있을 것으로 보여진다. 청지각적 평가를 통한 중재 전, 후의 변화는 전체 명료도가 8%, 음도 11.97%, 억양 10.12%, 강도 16.67%, 조음 6.97%의 유의미한 향상을 보였다. 또한, 전체 명료도 향상이 어떤 요인과 상관관계를 보이는지 확인한 결과, 조음(r=.97)과 가장 큰 상관관계를 보였으며, 음도(r=.80), 억양(r=.81), 강도(r=.51) 순으로 유의미한 상관관계를 확인할 수 있었다. 이는 파킨슨병 환자들의 말명료도를 향상시키기 위해서는 조음을 개선하는 것이 가장 중요하다고 볼 수 있다. 자가-지각을 통해 중재 전, 후의 말하기 노력을 확인하기 위해, 파킨슨병 환자들의 피로도를 주관적으로 평가한 결과, 유의미한 결과는 보이지 않았지만, 중재 후에 피로도가 13.1% 감소하였다. 따라서, Clear speech 중재 프로그램은 과도한 피로가 누적되지 않고도 말명료도 향상에 기여할 수 있다고 볼 수 있다. 연구논의: 파킨슨병 환자를 위한 치료 촉진법으로써의 LSVT와 Clear speech 중재프로그램은 궁극적으로 말명료도를 증가시키기 위해 실시될 수 있다. Dopamine deficiency in patients with PD affects nonverbal behaviors as well as speech. Patients with PD show motor problems, such as muscle rigidity, resting tremor, bradykinesia, abnormal postural reflex, and postural changes, and problems with balance and gait, all of which continue to deteriorate with the progression of the disease. Patients with PD have the speech characteristics of patients with hypokinetic dysarthria. Motor problems caused by reduced movement in patients with PD may affect some speech behaviors including breathing, vocalization, articulation, and resonance, and patients with PD are reported to have incomprehensible or unnatural speech. Because PD is a degenerative disease and deteriorated functions were believed to be irreversible, studies until the 1970s reported that speech therapy were not performed on patients with PD; however, after the 1980s, some studies(Ramig et al., 1988; Robertson & Thomson, 1984) reported that intensive speech therapy significantly improved speech and voice in PD patients. In particular, speaker-centered therapy teaches patients with PD to use behavioral strategies to improve speech intelligibility. Clear speech has been introduced as a technique that increases intelligibility of PD patients’ speech. It is fundamentally a combination of treatment techniques that increases vocal intensity while speech rate is reduced. This study aimed to indirectly compare the intervention with Lee SilvermanVoice Treatment (LSVT) and examine the differences between the two, and to determine whether Clear speech is an appropriate intervention for patients with PD. Methods: Fifteen patients with PD were participated in this study. The participants underwent an 8-session (2 session/week for 4 weeks) Clear speech intervention. Improvements in speech intelligibility were assessed by examining acoustical/aerodynamic, auditory-perceptual, and self-perceptual changes after the intervention in order to confirm whether the Clear speech intervention program can be used as a clinical tool for patients with PD. Originally, twenty patients were recruited, but five of them dropped out, resulting in a total of 15 participants (M=7, F=8). All participants were taking dopamine, had or had not received speech therapy more than two years ago, and had a Hoehn & Yahr stage score of 3 or below. The duration of the Clear speech intervention program was about 45–50 minutes. Each session was administered 1:1, and patients were given a homework for target voice practice. Baseline measurements were taken prior to the first session, and changes were measured after the four-week intervention. Acoustical, aerodynamic, auditory-perceptual, and self-perceptual assessments were performed to determine the changes after the intervention, and all values were presented as mean and standard deviation. Statistical analyses included test of normality, paired sample t-test, and Wilcoxon signed-rank test. Additionally, Pearson correlation analysis was also performed to observe the correlations between factors that may affect speech intelligibility in the auditory-perceptual assessment. Pre- and post-intervention data were analyzed with reference to a 95% confidence interval (p<.05). Results: With regard to the acoustical and aerodynamic changes after the Clear speech intervention, the participants showed increased habitual pitch and pitch range, reduced speaking rate, and increased breathing rate. Although there were no significant changes in intensity and average airflow rate, an increase in vocal intensity was not needed to improve speech intelligibility in patients with PD. in particular, Clear speech is focused on somewhat exaggerated speech or hyperarticulation; therefore, even without increased voice intensity, the intervention is speculated to improve speech intelligibility by resolving the problems with inaccurate articulation and rhythm. In the auditory-perceptual assessment, participants showed significant improvements in overall intelligibility, tone, intonation, intensity, and articulation. In addition, the improvement in overall intelligibility was most strongly correlated with articulation, followed by tone, intonation, and intensity. This result suggests that improving articulation is the most important factor for improving PD patients’ speech intelligibility. To examine speaking effort before and after the intervention through a self-perceptual assessment, the participants’ subjective fatigue was evaluated. Although statistically insignificant, fatigue decreased after the intervention. Therefore, the Clear speech intervention program can contribute to improving speech intelligibility without causing excessive fatigue. Conclusions: LSVT and Clear speech programs can be used for patients with PD to ultimately increase their speech intelligibility. LSVT is a voice treatment program that aims to increase voice intensity. The therapy focuses on vocalization to promote recovery of the weakened vocal abduction. LSVT is an intensive therapy that is administered four times a week for four weeks (16 sessions), and it helps patients to automatically have high vocal intensity in daily communications and maintain the treatment effects in the long term. Furthermore, it is a traditional treatment with a hierarchical approach, beginning with vowel lengthening, followed by vocabulary, phrase, sentence. In contrast, the Clear speech intervention program is administered twice a week for four weeks (8 sessions) and focuses on increasing speech intelligibility through hyperarticulation in conversations or connected speech. It is suggested that the Clear speech and LSVT programs should be chosen based on patients’ performance. In other words, speech characteristics varyamong patients with PD, with some patients have difficulty with speech rate while others have problems with voice intensity. Thus, instead of applying a one-size-for-all type of intervention for patients with PD, selecting and combining appropriate treatment techniques for each patient would contribute to improving speech intelligibility in such patients. Although there were no significant changes in intensity and average airflow rate, an increase in vocal intensity was not needed to improve speech intelligibility in patients with PD. in particular, Clear speech is focused on somewhat exaggerated speech or hyperarticulation; therefore, even without increased voice intensity, the intervention is speculated to improve speech intelligibility by resolving the problems with inaccurate articulation and rhythm. In the auditory-perceptual assessment, participants showed significant improvements in overall intelligibility, tone, intonation, intensity, and articulation. In addition, the improvement in overall intelligibility was most strongly correlated with articulation, followed by tone, intonation, and intensity. This result suggests that improving articulation is the most important factor for improving PD patients’ speech intelligibility. To examine speaking effort before and after the intervention through a self-perceptual assessment, the participants’ subjective fatigue was evaluated. Although statistically insignificant, fatigue decreased after the intervention. Therefore, the Clear speech intervention program can contribute to improving speech intelligibility without causing excessive fatigue. Conclusions: LSVT and Clear speech programs can be used for patients with PD to ultimately increase their speech intelligibility. LSVT is a voice treatment program that aims to increase voice intensity. The therapy focuses on vocalization to promote recovery of the weakened vocal abduction. LSVT is an intensive therapy that is administered four times a week for four weeks (16 sessions), and it helps patients to automatically have high vocal intensity in daily communications and maintain the treatment effects in the long term. Furthermore, it is a traditional treatment with a hierarchical approach, beginning with vowel lengthening, followed by vocabulary, phrase, sentence. In contrast, the Clear speech intervention program is administered twice a week for four weeks (8 sessions) and focuses on increasing speech intelligibility through hyperarticulation in conversations or connected speech. It is suggested that the Clear speech and LSVT programs should be chosen based on patients’ performance. In other words, speech characteristics varyamong patients with PD, with some patients have difficulty with speech rate while others have problems with voice intensity. Thus, instead of applying a one-size-for-all type of intervention for patients with PD, selecting and combining appropriate treatment techniques for each patient would contribute to improving speech intelligibility in such patients.
