국문초록 복강경하 질식 자궁적출술에서 비만이 산소화 및 환기에 미치는 영향 손희원 (마취통증의학 전공) (지도교수: 김 영 재) 의학과 인제대학교 대학원 목적: 비만 환자는 정상 체중 환자에 비해 산소화 및 환기에 문제가 있다. 비만 환자에서 복강내 CO2 주입으로 인한 기복시 가스교환 및 환기변수를 정상체중 환자와 비교 평가하고자 한다. 방법: 전신마취로 복강경하 질식 자궁적출술을 시행받은 정상군(n=22)과 비만군(n=23)을 연구하였다. 표준화된 마취 방법이 적용되었고 호기말 이산화탄소분압(ETCO2)이 45 mmHg 미만으로 유지되도록 일회 호흡량과 호흡수를 조절하였다. 심박수, 평균혈압, ETCO2, 동맥혈 이산화탄소분압(PaCO2), 기도압 및 동맥혈 산소분압(PaO2)을 기복 전(기관 삽관 3분 후), 기복 및 두부하강 체위 후 6, 12, 18 분, 그리고 CO2제거 및 앙와위 회복 후 6, 12, 18 분에 각각 측정하였다. 결과: 심박수, ETCO2, PaCO2 및 ETCO2-PaCO2는 두 군간 유의한 차이를 보이지 않았으나, 시간경과에 따라 차이를 보였다(p<0.001). 기도압, 폐포-동맥혈 산소분압차(A-aDO2)는 비만군에서 정상 체중군에 비해 유의하게 더 높았고(p<0.001), 시간경과에 따라서도 차이를 보였다(p<0.001). ABSTRACT The effects of obesity on oxygenation and ventilation during laparoscopic assisted vaginal hysterectomy Hee-Won Son (anesthesiology and pain medicine) (Advisor: Prof, Youngjae Kim, M.D.) Department of Medicine Graduate School, Inje University Objectives: Obese patients are more impaired in oxygenation and ventilation than normal weight patients. the purpose of this study was to compare changes of gas exchange and ventilatory variables between obese and normal weight patients during CO2 pneumoperitoneum. Methods: We studied normal weight group (n=22) and obese group (n=23) who underwent laparoscopic assisted vaginal hysterectomy under general anesthesia. A standarized anesthetic regimen was used, and end tidal CO2 (ETCO2) was maintained under 45mmHg by changing the tidal volume and respiratory rate. We measured heart rate (HR), mean blood pressure (MBP), ETCO2, PaCO2,, airway pressure and PaO2. Variables were measured before pneumoperitoneum (3 min after intubation), 6, 12 and 18 min after pneumoperitoneum and head down position, and 6, 12, and 18 min after removing CO2 and returning to supine position. Results: There was no significant difference in HR, ETCO2, PaCO2 and ETCO2-PaCO2 between two groups, but there was a significant difference as time process (p<0.001). Airway pressure and alveolar-arterial PO2 difference (A-aDO2) had significantly more increase in obese group compared to normal weight group, and were different as time process (p<0.001). PaO2 was significantly lower in obese group compared to normal weight group(p<0.001) and was different as time process (p<0.001). Conclusions: In obese patients undergoing laparoscopic assisted vaginal hysterectomy airway pressure and A-aDO2 is significantly increased than normal weight patients, and PaO2 is more impaired compared to normal weight patients.
