1. 국내 18F-FDG 제조 품목허가 기관의 품질관리 시험항목은 총 13개 이며 각 항목의 시험결과로 성상, 방사성이핵종, Endotoxin, Kryptofix, Aluminium 이온, 무균시험은 기준에 100% 적합하게 나타났다.
3. 따라서 18F-FDG 방사성의약품 품질관리 항목 총 13개 모두 시험판정 결과가 100% 적합한 것으로 나타났으므로 국내에서 제조 및 판매되고 있는 18F-FDG 방사성의약품은 PET-CT 검사를 위해 인체 내에 주입할 경우 안전하고 높은 화질의 영상을 획득할 수 있을 것으로 예측할 수 있다.
4. 품질관리 시험항목 이외의 기록에서 출하 승인자, 제조관리 책임자, 품질관리책임자, 제조관리자, 시험지시자의 경우는 기록을 하지 않는 경우가 높게 나타났다. 이는 18F-FDG 방사성의약품의 품질확보와 책임의 한계를 명확하게 하기 위해 품질관리 시험성적서에 위 항목을 기록하는 것으로 보완해야 한다.
5. 국내 8개 제조기관의 품질관리 시험성적서에 기록항목 존재여부를 조사한 결과 성상, 방사성이핵종, 방사화학적 순도, pH, Endotoxin, 무균시험의 기록항목 존재여부는 있다가 거의 100% 나타났고, Aluminium 이온 3.2%, 반감기 3.3%, Kryptofix 13.3%, 방사화학적 확인(RF값 측정) 34.2%, 실용량 40.2%로 기록항목이 있는 것으로 나타났으며 국내 제조기관 마다 18F-FDG 방사성의약품의 품질관리 시험항목이 차이가 있음을 알 수 있다. 미국약전 (USP : United States Pharmacopoeia) 유럽약전(EuPh : Europe Pharmacopoeia), 일본 관련법, 국내 관련법, 허가증의 품질관리 시험항목을 고려하여 국내 제조기관의 18F-FDG 방사성의약품 품질관리 시험항목을 통일시키고 품질관리 확립이 필요하다.
6. 18F-FDG 방사성의약품의 제조방법에 따라 품질관리 시험항목의 차이가 조금 있지만 국내 관련법 및 허가증의 품질관리 시험항목 기준을 고려할 경우 18F-FDG 방사성의약품 제조기관은 일반적으로 방사화학적 순도, 잔류용매 Ethanol, 잔류용매 Acetonitrile, pH, Kryptofix, 무균시험, Endotoxin, 반감기, 성상, 방사성 이핵종, 실용량, Aluminium 이온 총 12개 항목에 대한 품질관리 시험을 정기적으로 시행해야 한다. 그리고 제조된 방사성의약품이 18F-FDG가 맞는지 확인이 필요하며 이를 위해 Radio TLC Scanner 등을 이용하여 박층크로마토그래프법에 따라 방사화학적 순도시험을 시행하면서 방사화학적 확인시험(RF값의 측정 및 기록)도 병행할 것을 권장한다.

(Abstract) Although the usage frequency of 18F-FDG radiopharmaceuticals has been on a rapid increase in recent times, research on the quality control of domestic 18F-FDG radiopharmaceuticals is extremely inadequate, and the quality control test criteria differ amongst each manufacturing institution within the country. This study seeks to survey and analyze the quality control test criteria and results following the manufacture of 18F-FDG radiopharmaceuticals in Korea, in order to unify the quality control test criteria for 18F-FDG radiopharmaceuticals of Korean manufacturers, to contribute to the establishment of quality control, and to provide basic data on the quality control of 18F-FDG radiopharmaceuticals. On the basis of October 2009, this study surveyed and analyzed 1403 copies of quality control test results of complete 18F-FDG radiopharmaceutical products produced between 6 months of January and June 2009, from 8 institutions that allowed collection of quality control test results out of a total of 18 institutions authorized by the Korea Food & Drug Administration (KFDA) for 18F-FDG manufacture articles approval and declaration, using the SPSS statistical program. The results were as follows: Firstly, there were a total of 13 quality control test criteria of domestic 18F-FDG manufacture article approval institutions, and the test results of criteria on properties, radionuclide, endotoxin, kryptofix, aluminum ion, and asepsis proved to be 100% appropriate for standards. Secondly, the test results for numerical type criteria on half-life at 109.7047±1.42087 (min), radiochemical confirmation (RF value measurement) at 0.4770±0.05494, radiochemical purity at 98.6216±1.53287(%), actual volume at 1667.00±1250.845 (mCi)/16.00±0.000 (ml), residual solvent ethanol at 70.96±10.2532 (ppm), and residual solvent acetonitrile at 110.89±13.2543 (ppm) pH 6.612±0.4946 were also proved to be 100% appropriate for standards. Thirdly, therefore, as all 13 criteria of the quality control criteria of 18F-FDG radiopharmaceuticals turned out to be 100% appropriate, those manufactured and sold in Korea can be predicted to achieve safe and high-quality images upon injecting within the human body for PET-CT testing. Fourthly, as for records other than those of quality control test criteria, the values were higher on not recording in regard to the shipping authorizing personnel, chief of manufacture control, chief of quality control, manufacture manager, and test manager. This must be complemented to recording the aforementioned criteria on the quality control test results to clarify the limitations of responsibility and to achieve quality of 18F-FDG radiopharmaceuticals. Fifthly, as a result of surveying the existence of recording criteria on the quality control test results of 8 Korean manufacturing institutions, almost 100% turned out to have recording criteria for tests on properties, radionuclide, radiochemical purity, pH, Endotoxin, and asepsis. Recording criteria were present for aluminum ion at 3.2%, half-life at 3.3%, kryptofix at 13.3%, radiochemical confirmation (RF value measurement) at 34.2%, and actual volume at 40.2%. Also, there were differences in the quality control test criteria of 18F-FDG radiopharmaceutical amongst all domestic manufacturing institutions. Therefore, there is the need to establish quality control and to unify the quality control test criteria of 18F-FDG radiopharmaceutical of domestic manufacturing institutions in consideration of the United States Pharmacopoeia (USP), Europe Pharmacopoeia (EuPh), related laws of Japan, related laws of Korea, and the quality control test criteria of license permits. Lastly, although there were some differences in the quality control test criteria according to the manufacturing methods for 18F-FDG radiopharmaceuticals, it is general for the 18F-FDG radiopharmaceutical manufacturing institutions to regularly conduct the 12 quality control test criteria on radiochemical purity, residual solvent ethanol, residual solvent acetonitrile, pH, kryptofix, asepsis test, Endotoxin, half-life, properties, radionuclide, actual capacity, and aluminum ion when considering for related laws in Korea and the quality control test criteria standards of license permits. Also, it is recommended to add the criterion of radiochemical confirmation (RF value measurement) to confirm whether the 18F-FDG of the manufactured radiopharmaceutical is correct.
