국문초록 동맥 내 혈압 측정을 통한 국내 시판 중인 자동 상박 혈압계의 신뢰도 평가 김 수 홍 (내과학 전공) (지도교수 : 김 동 수) 의학과 인제대학교 대학원 목적: 고혈압의 진단과 치료를 위해 혈압을 정확히 측정하는 것은 중요하다. 자동 상박 혈압계를 사용한 자가 혈압 측정이 보편화 되고 있으며 다양한 자동 혈압계가 시판 중에 있으나, 측정의 정확성에 대한 주의가 필요하다. 이 연구에서는 상완동맥 및 상행대동맥에 대한 동맥 내에서 측정한 혈압과의 비교를 통하여 자동 상박 혈압계의 신뢰도를 평가하고자 하였다. 방법: 2009년 1월부터 10월까지 부산보훈병원 순환기내과에서 관상동맥 조영술을 시행한 환자 중 참여 동의서를 획득한 265명을 대상으로 하였다. 관상동맥 조영 도관을 이용하여 우측 상완동맥과 상행 대동맥 원위부에서 직접 측정한 동맥 혈압 및 맥박과 국내 시판 중인 6개의 자동 상박 혈압계를 각 대상자에게 순차적으로 3개씩 적용하여 얻은 좌측 상완동맥의 혈압과 맥박을 비교하였다. 자료 분석은 SPSSⓇ 프로그램(version 16.0, Chicago, Illinois, USA)을 이용하여 각 측정법의 측정값을 paired t-test와 Pearson 상관분석으로 비교하고, Bland-Altman 분석을 실시하였다. 혈압의 차이가 5 mmHg 미만이고, paired t-test의 P값이 0.05보다 크고, 상관계수가 0.7보다 큰 경우를 임상적으로 신뢰할 만한 것으로 판정하였으며, P < 0.05인 경우 통계적으로 의미 있는 것으로 판정하였다. 결과: 6개 중 각 3개씩 자동 상박 혈압계를 적용한 대상 환자의 성 연령 분포와 체격지수, 병력은 차이가 없었다. 상완동맥에서 직접 측정한 결과와의 비교에서는 수축기 혈압은 3가지 (Device A, C, F), 이완기 혈압은 4가지 (Device A, B, E, F)에서 5 mmHg 이상의 차이를 보였으나, 이완기 혈압에서 5 mmHg이하의 차이를 보인 2가지는 p값이 0.05미만으로 나와 신뢰할 수 없었다. 상관분석에서 수축기 혈압은 6가지 모두 좋은 상관관계를 보였으나, 이완기 혈압은 2가지에서 임상적으로 신뢰할 수 없는 상관관계를 보였다. 상완동맥 측정값과 비교 시 수축기 혈압은 3가지 (Device B, D, E)에서 신뢰할 만한 결과를 보였고, 이완기 혈압은 신뢰할 만한 결과를 보이는 것은 없었다. 상행 대동맥에서 직접 측정한 결과와의 비교에서는 수축기 혈압은 2가지 (Device C, F), 이완기 혈압은 1가지 (Device A) 에서 5 mmHg 이상의 차이를 보였으나, 이완기 혈압에서 5 mmHg 이하의 차이를 보인 5가지 중 3가지는 p값이 0.05미만으로 나와 신뢰할 수 없었다. 상관계수와 Bland-Altman 분석 결과도 혈압계간 차이를 보였다. 상행 대동맥의 측정값과 비교 시 수축기 혈압은 4가지 (Device A, B, D, F)에서, 이완기 혈압은 2가지 (Device C, D)에서 신뢰할 만한 결과를 보였다. 결론: 일부 자동 상박 혈압계의 측정값은 동맥 내 혈압 측정값과 유의한 차이를 나타내어 측정의 신뢰성에 문제가 있음을 보여 주었다. 고혈압 관리에는 무엇보다도 정확한 혈압 측정이 중요하며, 최근 자동 상박 혈압계를 이용한 자가 혈압 측정이 빠르게 보편화 되고 있으므로 이를 통한 측정의 정확성에 대한 주의가 필요하겠다. 자동 상박 혈압계를 사용함에 있어 고혈압의 진단보다는 혈압의 변화양상을 평가하고, 치료의 효과를 보는 것이 더 중요할 것으로 생각되며, 자동 상박 혈압계를 이용한 혈압 측정의 신뢰성에 대한 보완 연구가 필요하겠다. Nevertheless automated blood pressure measuring instruments (ABPMI) have been being popular and widely used for the self monitoring of blood pressure, the accuracy of this measurement is still a matter of debate. This study was conducted to assess the reliability of the measurement by ABPMI comparing with intra-arterial measurement. Methods: Study subjects were the 265 patients who received coronary angiography and voluntary participated with informed consent at Busan Veterans Hospital from January to October 2009. The blood pressures measured by three of the six ABPMIs at the left brachial artery were compared with those by intra-arterial measurement at right brachial artery and ascending aorta. The paired t-test, Pearson correlation analysis and Bland-Altman analysis were done using SPSSⓇ version 16.0 for WindowsⓇ (Chicago, Illinois, USA). When the difference of blood pressure between the two measurements showed within 5 mmHg, P value by paired t-test > 0.05 and correlation coefficients > 0.7 , it considered as 'clinically reliable'. P < 0.05 was considered statistically significant. Results: The characteristics and disease pattern of the subjects who applied the three of the six ABPMIs had no differences. As the results of comparison with intra-arterial brachial artery measurement, there were some discrepancies in systolic blood pressure (SBP) of the three ABPMIs (Device A, C, F) and diastolic blood pressure (DBP) of the all ABPMIs. As the results of comparison with intra-arterial ascending aorta measurement, there were some discrepancies in systolic blood pressure (SBP) of the two ABPMIs (Device C, F) and diastolic blood pressure (DBP) of the four ABPMIs (Device A, B, E, F). Conclusion: The blood pressures measured by part of the ABPMIs showed the discrepancies with the intra-arterial measurements.
