MATERIALS and METHODS: Thirty-nine patients with the pneumomediastinum were retrospectively evaluated. All CT scans included thin section CT, and were obtained within 24 hours after the presentation of symptom or procedure. We classified the causes of the pneumomediastinum as traumatic (n=16), idiopathic (n=13), and iatrogenic (n=10). Also, we classified the level of the bronchovascular bundle as main (from carina to second carina), lobar (from second carina to the distal portion of lobar bronchi) and segmental (below the level of lobar bronchi). On the CT scans, linear or round extra-luminal air densities along the bronchovascular bundles were evaluated. RESULTS : At the level of lobar and segmental, extra-luminal air density was seen in idiopathic (100%, 84.6%) and traumatic (31.3%, 37.5%), however, was not seen in iatrogenic in all cases. Extra-luminal air density in both lungs at the level of lobar and segmental was more common in idopathic (61.5%, 45.5%) than in traumatic (40%, 16.7%), however, it did not show statistical significance. CONCLUSION : The Macklin effect that is helpful in the differentiation of the pneumomediastinum should be evaluated at the lobar level below the second carina.
