The purpose of this study is to determine, by conducting an 8-week hippotherapy, the effects on the upper extremity function, balance and ambulation of children with intellectual disabilities. 45 children with intellectual disabilities were randomly divided into 3 groups of 15 children in each group and participated in the study: hippotherapy group, horseback-riding machine group and control group. As measurement methods, Vineland Social Maturity Scale was used for sociability measurement and ambulation analysis system, AP1105 (GAITrite EWPU), was used for ambulation measurement. A 3-D motion analyzer was used to measure upper extremity function, and balance ability measurement system was used to measure balance ability. As an intervention method, a program from the American Hippotherapy Association was modified and enhanced, and it was administered on the hippotherapy group twice a week for a total of 8 weeks with each session lasting 45 minutes. The horseback-riding machine group was administered with a horseback-riding machine twice a week for a total of 8 weeks with each session lasting 30 minutes The analysis of the results was performed using a dual-position analysis of variance to verify the balance between the exercise groups and intervals, and ambulation, upper extremity function and social maturity. Also analyzed was statistical significance between the exercise groups. The study results showed the following: in the comparison of 3 types of exercises and treatment for children with intellectual disabilities, as compared to the control group, the use of horseback-riding machine showed more improvement in step length, stride length, step time and social maturity regarding ambulation, and showed reductions within the tolerance range regarding balance in eyes-open state and upper extremity function; and the use of hippotherapy showed more positive effects, compared to the use of horseback-riding machine, regarding balance, ambulation, upper extremity function and also social maturity. In conclusion, hippotherapy could provide positive effects for children with intellectual disability not only on sociability, but also on ambulation, balancing ability and upper extremity function. If hippotherapy-based programs that are systematic and customized for disorder characteristics can be developed for children with other disabilities, it is deemed that these programs can be proposed as more effective treatment methods not only for children with intellectual disability, but also for children with other disabilities. As such, through a continued and long-term research, in the future there need to be studies with a variety of additional variables, along with studies for helping the rehabilitations of children with intellectual disability and studies on upper extremity function, balance, ambulation social maturity
