According to the studies on risk factors of obesity in abroad, it has been reported that obesity causes several diseases related to low back pain. Low back pain which occurs frequently across all age groups and all occupational categories is one of the most common musculoskeletal disorders. It is the disease which the high economic and social costs are required to treat. The purpose of this study is on adult patients with low back pain and abdominal obesity in Korea, and is to identify associations between exercise programs for low back pain, such as Aerobic exercise and MacKenzie & Williams exercise, and physical changes in pain intensity, muscle strength and weight loss. A total of 60 adult men and women were selected for this study. In accordance with individual Body Mass Index(BMI), patients were divided into the categories of overweight, obesity and extreme obesity. After personal information(for example, gender, physical figures and occupation) was identified, questions about the specific symptoms and duration of pain, treatment methods, duration of treatment, time of exercise, spending time in a chair and spending time on a computer were investigated through questionnaires. Besides, based on Oswestry Disability Index(ODI) outcomes, the 12-week intensive program of the combined McKenzie & William exercise and Aerobic exercise(i.e. running, cycling) was designed and prescribed to perform an hour exercise at a time, three times a week(divided into Mon-Wen-Fri team and Tue-Thu-Sat team). 12 weeks later, the physical changes was observed. There were a lot of overweight in both men and women before conducting the exercise program, as well as statistically significant association between obesity and symptoms, such as abdominal obesity and low back pain: the more obese people were at higher risk for abdominal obesity and low back pain. On the other hand, after 12-week program, the results showed that out of lots of variables related low back pain(including abdominal obesity, extreme obesity, low back pain and lifetime prevalence), there was more likely to have a causal relationship between low back pain and abdominal obesity, other diseases. In addition, it demonstrated that McKenzie & William exercise had excellent effect on abdominal obesity and weight loss. In this study, subjects were those who are considered to be overweight or obese from adult men and women aged 20 to 40 years residing in seoul. 10 men and 10 women were assigned to each group for three exercise programs. There are several limitations in this study. First, it is difficult to generalize the results of the study to the entire population. Second, since several factors, such as disposition, circumstances, dietary therapy and diet of subjects, was not considered in the study, it could be mistakenly assumed that only exercise programs affected weight loss. Third, analysis of questionnaire answers was based on subjective responses from the limited number of participants, because high-tech equipments such as medical devices or scientific instruments were not used. As a result of the exercise intervention program, the mean weight and mean Waist-hip ratio for men decreased from 82.16kg to 77.21kg, and from 0.93 to 0.91 respectively. In the case of women, the mean weight decreased from 71.40kg to 64.56kg. The mean obesity index and mean Waist-hip ratio decreased from 26.96 to 24.44 and from 0.88 to 0.86 respectively. In spite of these findings, further prospective studies with larger sample sizes and more diverse variables will be needed to confirm the causal relationship between low back pain and abdominal obesity or weight.
