It is important to improve treatment adherence in patients with Alzheimer’s disease, due to the gradual increase in the number of patients and the burdens that come with them. The primary aim of this study was to assess the medication adherence of Alzheimer’s patients using Medication Event Monitoring System (MEMS). The data was compared with the data obtained through pill count, clinician rating scale, and patient self-report. The secondary aim of the study was to analyze the relationships between adherence and other clinical parameters, such as sociodemographic and clinical variables, which include cognitive function, and social support.
Methods
In a four-week period, 33 outpatients, who were at least 65 years old and diagnosed with Alzheimer’s disease were monitored. All patients took cognitive enhancers. The percentage of doses taken on schedule was used to assess MEMS adherence, whose threshold was defined as 80% of adherence. Medication adherence was also assessed by using pill count, clinician rating scale, and patients self-report. Agreement among these adherence measurements was evaluated. In addition, sociodemographic variables, Mini-Mental Status Examination-Korean version, Clock Drawing Test, Korean-Dementia Rating Scale conceptualization similarity questions, Clinical Dementia Rating, Brief Questionnaire form of the Neuropsychiatric Inventory, and Korean version of Duke-UNC Functional Social Support Questionnaire were evaluated. The relationship between these clinical variables and adherence variables was assessed.
Results
The mean values for the various measurements of adherence were as follows: MEMS 72.50%, pill count 82.28%, clinician rating scale 5.24, and self-report 87.9%. The rates of adherence group were as follows: MEMS 51.5%, pill count 82.8%, clinician rating scale 82.8%, and self-report 87.9%. The Kappa coefficients were 0.382 (pill count vs. MEMS), 0.382 (clinician rating scale vs. MEMS) and 0.256 (self-report vs. MEMS). The rates of adherence group for MEMS were higher in men than women. Except for gender difference, no variables were correlated with adherence variables.
Conclusions
Adherence as measured by pill count, clinician rating scale, and patient self-report showed discrepancy with MEMS adherence in patients with Alzheimer’s disease. In this study, no variables except gender difference were correlated with adherence, due to caregiver’s help.
