This study was conducted to review the side-effects and effects on health insurance finances after the implementation of selling OTC drugs outside of pharmacies. Methods: The occurrence of sales and side-effects, and changes in health insurance finances after the implementation of selling OTC drugs outside of pharmacies were investigated from November 2012 to March 2013. Data was collected from the Korea Institute for Health and Social Affairs and the Korea Institute of Drug Safety and Risk Management. As for foreign precedent data, secondary data collected from Japanese data from PMDA, Japanese Ministry of Health, Labour and Welfare, and Japan's OTC Drug Site was used. Results: The number of OTC drugs sold after the implementation of the system, from November 15th 2012 to late March of 2013, was a total of 3,282,216. The side-effects of drugs have a tendency of increasing from 2010, and in the first two quarters of 2013 showed an increase of 98.8% than last year's first two quarters. Ten items among the 12 items of OTC drugs had side-effects, and 4 of them had severe side-effects. After the implementation of the system, the insurance pay disbursed by the National Health Insurance Corporation did not decrease from the first half of last year but rather increased 727.6 billion won which seemed to show that it had no effect on health insurance finances. Also, in the case of pharmacy insurance expenses, 197.2 billion won decreased from last year which seems to be due to the effects of lowering the price of medicine en bloc in April 2012. Medical institution expenses increased 1 trillion 30.9 billion won which seems to be due to the increase of medical usage of elders over 65.
