2. ABSTRACT These days, the need for research on the families of liver transplant patients is being raised in the field of medical social work practice for domestic organ transplantation. Domestically, the number of liver transplants has been increasing every year; the number of living-donor liver transplants is now more than three times that of brain-dead-donor liver transplants. Approximately 84% of living donors are blood relatives of the recipients; more than 60% are the recipients’ children. Considering these circumstances, it has been suggested by many that there is a need for research that is focused specifically on the stress experienced by liver transplant recipient families, in order to sustain the recipients’ life and health, improve the quality of life of patients’ families, overcome family crisis, and ultimately prevent a family breakdown. The reality is, however, that empirical research on family stress has not been accomplished because the concepts related to the stress of organ transplant recipient families are unclear and there has been no method of measuring the stress. Thus, this research introduces a reliable and adequate measure for determining the stress level of liver transplant recipients' families, which was developed to solve the aforementioned problems and promote research on the families’ stress. Moreover, through this research, we attempt to provide important practical evidence to lay the groundwork for later psychological, medical, and political support for liver transplant recipients' families in the field of medical social work practice. This study employs both qualitative and quantitative research methods to develop a measure of the stress of liver transplant recipients' families. First of all, we consulted a range of literature, conducted in-depth interviews with liver transplant recipient families, and analyzed their memoirs to conceptualize their stress and derive its component factors. Questions for each component factor were developed based on the related literature, in-depth interviews with the persons in question, their memoirs, and the researcher’s clinical experience. We established 53 preliminary questions after validity inspection by experts in the field. This research was conducted from March 22 to August 22, 2013. The subjects selected are family members who are chief care-givers older than 20 and living together with recipients who have undergone a liver transplant within the last 5 years at Hospital A in Seoul, which performs the largest number of liver transplantations in Korea. The participants were limited to those who agreed to take part in this research. Data collection was performed through a questionnaire approved by the Institutional Review Board (IRB) of Hospital A. The researcher met in person with family members of liver transplant recipients at the outpatient clinic of Division of Hepato-Biliary & Pancreas Surgery, explained the objective and purpose of this research, and distributed and collected the questionnaire after obtaining written consents from them. A total of 450 copies of the questionnaire were distributed and 416 copies were collected. Once insincere answers were excluded, a total of 403 copies was used for analysis. For data analysis, we performed a Kaiser-Meyer-Olkin (KMO) measure of sampling adequacy and Bartlett’s test of sphericity using Statistical Package for the Social Science (SPSS) ver. 19.0, and we also used exploratory factor analysis and stepwise regression analysis. The results of the analysis are as follows: First of all, through exploratory factor analysis, we abstracted a total of 39 questions loading each of 4 stress component factors, such as ‘a psychological and social stress factor,’ ‘a family stress factor,’ ‘an emotional stress factor for the donor’ and ‘an economic stress factor.’ Second, reliability was confirmed through internal consistency index and standard error of measurement. The overall Cronbach’s alpha of this measure was .96; checking the reliability indexes of sub-factors, ‘psychological and social stress’ was .95, ‘family stress’ .95, ‘emotional stress for the donor’ .83, and ‘economic stress’, .83, which indicates a relatively high level of reliability. The reliability index determined through standard error of measurement was 6.68, which also means ‘fairly reliable.’ Therefore, the reliability of the measure of the stress of liver transplant recipient families was secured. Third, convergent validity was evaluated to verify the validity of this measure. Above all, validity was verified through correlations between K6 & K10 and social support, in order to analyze the correlation between two measures of the same concept. Correlations between the total stress score of liver transplant recipient families and the total score of K6 & K10 appeared to be statistically meaningful; correlations between K6 & K10 and sub-factors of stress measurement of liver transplant recipient families also appeared to be positive, so convergent validity was verified. Correlation index between the total stress score of liver transplant recipient families and the total score of social support was -.402 (p<.01) and was statistically meaningful; correlation between sub-factors of social support and sub-factors of stress measurement of liver transplant recipient families also appeared to be negative and statistically meaningful. Thus, convergent validity was secured. Fourth, the most ideal cutting score (122) of the measure developed in this research divided the stress levels of liver transplant recipient families. 122 out of all subjects scored more than 122, which means 30.3% of all.
