본 연구는 CHEER(Children's Health Environmental Researth)자료를 활용하여 어린이들의 비스페놀 A, 프탈레이트 노출이 치아우식증에 미치는 영향을 알아보기 위해 시행되었다.

This study was conducted to investigate the impact of bisphenol-A and phthalate exposure on dental caries in children, using data from the Children’s Health Environmental Research(CHEER). The subjects included 1,445 children between 7 and 13 in age, for which bisphenol-A and phthalate (MEHHP, MEOHP, MnBP) exposure tests and dental examinations were conducted. The subjects were divided by gender, and the dental examinations looked into the decayed tooth surfaces, filled surfaces, and DMFS tooth surfaces of their deciduous teeth and permanent teeth. R-3.0.2 was used for statistical analysis. The concentrations of bisphenol-A and phthalate in the subjects’ urine were divided into quartiles, and risk for caries in deciduous teeth and permanent teeth except for DMFS teeth was analyzed. The group with the highest MEHHP concentration in urine had 2.21 times (95%C.I:1.07-4.56) higher risk for caries than that of the lowest group, and continuous concentrations of DEHP also showed statistically significant association. As to genders, in boys the group with the highest bisphenol-A concentration in urine had the highest risk for caries (2.97 times, 95% C.I:1.03-8.94) than that of the lowest group, in girls the group with the third highest MEHHP and MEOHP concentrations in urine had 3.69 times (95%C.I:1.45-9.39) and 2.68 times (95%C.I:1.05-6.84) higher risk for caries than that of the lowest group, respectively. Risk for caries in deciduous teeth and permanent teeth was analyzed. In girls, the group with the second highest MnBP concentration in urine had 1.87 times (95%C.I:1.06-3.29) higher risk for decayed tooth surfaces in their deciduous teeth, and the group with the third highest bisphenol-A concentration in urine had 1.85 times (95%C.I:1.85-3.16) higher risk for DMFS in their deciduous teeth than that of the lowest group, respectively. In boys, the group with the highest MnBP concentration in urine had 1.83 times (95%C.I:1.04-3.23) higher risk for decayed tooth surfaces in their deciduous teeth, and the group with the highest MnBP concentration in urine had 3.70 times (95%C.I:1.51-9.07) higher risk for decayed tooth surfaces in their permanent teeth than that of the lowest group, respectively. In girls, the group with the second and third highest MnBP concentration in urine had 2.13 times (95%C.I:1.15-3.98), and 1.88 times (95%C.I:1.08-3.31) higher risk for decayed tooth surfaces in their permanent teeth than that of the lowest group, respectively. Also, risk for DMFS in permanent teeth in girls had statistically significant association with MEHHP and DEHP concentrations in urine. This study showed that bisphenol-A and phthalate concentrations in urine are risk factors for tooth caries. While the bisphenol-A concentration had more influence on caries in deciduous teeth compared to that on permanent teeth, the MnBP concentration was more associated with caries in permanent teeth. This study is a cross-sectional study that used the CHEER data and has the limitation of unavailability to identify the temporal order of the incidence of dental caries, which calls for more detailed subsequent studies to follow up bisphenol-A and phthalate concentrations to understand the mechanism through which they affect dental caries in children, and plans and measures to prevent them from affecting oral health.
