본 연구의 목적은 국내 전 연령의 프탈레이트의 인체 노출의 수준을 파악하고 프탈레이트와 비스페놀 A 노출에 영향을 미치는 요인을 분석하는 것이다. 연구대상자는 유해물질 안전관리 통합노출 평가기반 연구사업단(Korean Research Project on the Integrated Exposure Assessment to Hazardous Materials for Food Safety, KRIEFS)이 2010년부터 2011년까지 구축한 코호트를 이용하여 12시간 소변 내 프탈레이트 대사산물과 비스페놀A를 측정한 2,498명중 크레아틴 농도 값이 없는 7명을 제외한 2,491명(남자 1,223명, 여자 1,268명)이다. 조사대상자에게 수집한 소변시료와 건강검진, 식이설문, 식품 분석 결과를 근거한 식이 노출량을 근거로 인체 노출 수준에 영향을 미치는 요인을 인구학적 요인(Demographic factors)과 개인의 사회경제적 요인(Socioeconomic factors), 환경적 요인(Environmental factors), 식이 요인(Dietary factors), 생활 행동 요인(Health behavior factors)과 지역적 요인(Regional factors), 여섯 가지로 구분하여 분석하였다. 통계분석은 통계프로그램 R 2.15.2를 사용하였다. 조사대상자의 일반적 특성에 따른 프탈레이트와 비스페놀A 농도 분포는 기술 통계를 실시하고 그룹별 비교는 Wilcoxon test와 Kruskal-Wallis test를 하였다. 인체 노출 수준과 각 요인간의 관련성 분석은 회귀분석(Regression analysis)과 다중회귀분석(Multiple regression analysis)을 실시하였다. 프탈레이트, 비스페놀A 인체 노출 수준에 영향을 미치는 요인은 구조방정식(Structural equation modeling)을 이용하여 분석하였다. 전체 조사대상자의 뇨중 비스페놀A 기하평균 농도(ug/g cr)는 여성(2.43)이 남성(2.19)보다 높았다(p 0.01). 연령별로는 0-5세(4.84), 6-11세(3.0)집단이 청소년, 성인보다 높았다(p<0.0001). 교육기간이 길수록 비스페놀A 농도가 높았으며 (교육기간이 13년 이상인 집단의 기하평균농도는 2.57, 12년 2.17, 12년 미만 1.53 p<0.0001), 가구소득은 소득별 4개 집단에서 중하위권 집단이 2.6으로 하위권(1.7), 중위권(2.4), 상위권(1.87)보다 높았다(p<0.0001). 식이행동 요인으로는 하루에 1회 이상 캔 식품을 섭취하는 집단(19.1)이 한 달에 1회 미만(2.26), 한 달에 1회(2.43), 1주일에 1회(2.19),하루에 1회이상(2.43)보다 높았다(p<0.0001). 조사대상자의 뇨중 프탈레이트(DEHP)의 기하평균 농도(ug/g cr)는 남성(104.1)이 여성(100.5)보다 높았다(p<0.0001). 연령별로는 12-19세 (151.13)집단이 0-5세(105.9), 6-11세(108.1), 20-64세(85.9), 65세이상(95.7)보다 높았다(p <0.0001). 사회경제적 요인으로 농촌지역 거주자(117.9)가 대도시(104.72)나 중소도시(89.4)에 비해 높았다(p<0.0001). 교육수준은 교육기간이 13년 이상 집단이 높았다(114.9, 12년 94.2, 12년 미만 69.1, p<0.0001). 가구소득은 소득별 4개 집단에서 중하위권 집단이 115.0으로 중상위권(103.7), 하위권(75.9), 상위권(85.0)보다 높았다(p<0.0001). 식이노출요인으로 최근 1년 동안 평균 외식 및 배달음식 횟수별 프탈레이트(DEHP) 농도 분포는 한 달에 1회 이상 외식 및 배달음식을 먹은 집단(137.2)이 한 달에 1-3회(102.1), 한 달에 1회 미만(101.0), 1주일에 1-3회(97.3)보다 높았고(p0.04) 환경적 요인으로 현재 거주하고 있는 집에서 가장 가까운 도로와의 거리가 50미터 이내(108.22)인 집단의 농도가 가장 높았다(500미터 이내 106.8, 500미터 이상 95.9, 바로 옆 89.2, p0.03). 구조방정식(structural equational modeling)을 이용한 확인적 요인 분석 결과 비스페놀 A 가설 모형에서 통계적으로 유의하게 나타난 경로는 사회경제적 잠재변수(latent variable)와 식이노출량 관측변수(observed variable)였고 성별, 나이, 지역, 환경 잠재변수는 유의하지 않았다. 사회 경제적 요인이 증가할수록 비스페놀A 농도가 유의하게 감소하였고(β-0.073, p0.001), 단위체중당 식품을 통한 비스페놀A 1일 노출량이 증가할수록 비스페놀A 농도값도 유의하게 증가하였다(β0.207, p0.001). 프탈레이트(DEHP)의 가설 모형에서 통계적으로 유의하게 나타난 경로는 나이와 사회경제적 잠재변수, 식이노출량 변수였고 성별, 지역 환경 요인은 유의하지 않았다. 나이가 증가할수록 DEHP 농도가 유의하게 감소하였고(β-0.064, p0.001), 사회경제적 요인이 증가할수록 프탈레이트(DEHP)농도는 유의하게 감소하였다(β-0.0088, p0.001), 단위체중당 식이를 통한 DEHP 1일 노출량이 증가할수록 DEHP 농도값도 유의하게 증가하였다(β0.347, p0.001). BPA, DEHP모두 식이 요인이 다른 요인(사회 경제적, 환경요인적, 성별, 나이)들에 비해 상대적으로 인체노출에 유의하게 영향을 주는 것으로 나타났다. 향후 프탈레이트 및 비스페놀A 노출에 대한 지속적인 연구와 프탈레이트 및 비스페놀A 노출 감소를 위한 국가적 차원의 노력, 어린이와 민감계층에 대한 예방정책 수립이 필요하다.

Purposes of this study are to identify the level of phthalate and BPA’s exposure to a human body at all ages across the country, and to analyze factors that have influences on phthalate and BPA’s exposure. Study subjects were 2,491 persons (male: 1,223, female: 1,268) who measured phthalate metabolite and BPA in 12 hours urine using the cohort based on the whole the nation established in Korean Research Project on the Integrated Exposure Assessment to Hazardous Materials for Food Safety (KRIEFS) from 20120 to 2011. With regard to factors having influences on the level of exposure to a human body by focusing on estimation of dietary exposure as a result of evaluation of urine samples, physical examination, questionnaire on dietary, and food analysis results collected from the subjects, analysis has been made by six categories Demographic factors, Socioeconomic factors, Environmental factors, Dietary factors, Health behavior factors, and Regional factors. Statistical analysis were performed using R statistical software Version 2.15.2. Phthalate and BPA concentration by general characteristics of this study subjects used technological statistics, Wilcoxon tests and Kruskal-Wallis tests. For a level of exposure to a human body and its relevance of each factor used multiple lenear regression analysis. For paths having impacts on the level of phthalate and BPA’s exposure to a human body used a structural equation modeling analysis. As a result, urinary BPA level(ug/g cr) were showed a higher level in the male group (2.19) than the female group (2.43). From the age-specific perspective, age groups of 1–5 (4.84), 6-11 (3.0) showed higher levels than those of adolescent and adults. (p<0.0001) For higher educated people, they showed higher BPA level (more than 13 years, 2.57, p<0.0001); a middle-lower group out of four income level groups showed the highest of 2.6; as for analysis on intake frequency of canned-type food, a group having more than one canned food showed the highest (3.19, p<0.0001). urinary DEHP level(ug/g cr) showed a higher level in the male group of 104.1 than that of female of 100.5(p <0.0001).
