Introduction : A coronary artery lesion (CAL), associated with Kawasaki disease (KD) is unclear to be affected or unaffected by the presence of arthritis. Eczematoid lesion is transient itching and crusting or scaling phenomenon in patients with acute KD and may result from some factors including intravenous immunoglobulin (IVIG) and viruses . But few studies have reported the correlation between eczematoid lesion and CAL in patients with KD. This study intends to identify coronary outcomes for patients with the two clinical phenotypes in KD. Materials and methods : From August, 2006 to December, 2013, this single-center, retrospective review of medical records of 220 patients diagnosed with KD and infused with IVIG during hospitalization at Kyung Hee University Hospital at Gandong was performed. 52 patients with eczematoid lesion in KD and 6 patients with arthritis were selected and evaluated for clinical characteristics, including fever duration, CAL , associated viral or bacterial infection and compared with controls. All statistical analyses were performed using SPSS ver 21.0 (IBM, New York, USA). A P value of less than 0.05 was considered statistically significant . Results : The prevalence of eczematoid lesion was 23.6% (52/220). The children with eczematoid lesion were significantly older, and had significantly longer fever duration, significantly higher prevalence of CAL than controls (P=.000, P=.041, P=.033, respectively). Pyuria was significantly more frequent in patients with eczematoid lesion than controls (P=.000). The prevalence of arthritis was 3% (6/220). The children with arthritis were significantly more frequently infected with viruses than those without arthritis (P=.018). The viruses that isolated from the patietnts with arthritis were influenza A and B virus, rhinovirus in 1, parainfluenza 2, metapneumovirus in 1, coronavirus OC43 in 1. Two of these infected with viruses had CAL, One was coinfected with parainfluenza 2 and metapneumovirus, another was infected with coronavirus OC43. Conclusions : Patients with eczematoid lesion were at significantly high risk of developing CAL.
