본 연구에서는 아목시실린(Amoxicillin trihydrate), 클라불란산(Clavulanic acid) 합제의 개에서의 Escherichia coli (E. coli)성 감염증에 대한 효능과 안전성을 확인하고자 개에서 분리된 E. coli(표준균주 1주, 야외균주 3주)에 대하여 Minimum Inhibitory Concentration Assay (MIC 시험)를 실시하고 Fractional Inhibitory Concentration Index (FIC index) 값을 산출하여 복합의 의의를 확인하였다. 아울러, 개의 방광에 E. coli를 접종하여 요도염을 유발한 후 아목시실린:클라불란산(4:1) 합제의 치료효과를 조사하고 안전성 시험을 실시하여 다음의 결과를 얻었다. 공시한 E. coli 4개 균주에 대한 아목시실린의 MIC 시험결과 E. coli의 야외균주 3주에서는 6.25 µg/mL 이하였으며, 표준균주는 12.5 µg/mL이었다. 공시한 E. coli 4개 균주에 대한 클라불란산의 MIC 는 62.5 µg/mL으로 단독처리 시 효과가 미약함을 알 수 있었다. 공시 균종에 대한 아목시실린:클라불란산(4:1) 복합제제의 MIC 시험결과, 2.5 + 0.625 µg/mL(아목시실린 + 클라불란산) 이하로서 아목시실린 단독처리의 경우보다 1.25∼10배 항균효능이 증가하였고, 클라불란산 단독처리의 경우보다 약 100∼200배 증가하여 복합처리 시 효과가 증가하는 것을 알 수 있었다. 아목시실린과 클라불란산의 여러 농도에서의 병용효과를 Checkerboard 방법에 의해 확인한 결과 E. coli에 대한 아목시실린과 클라불란산의 FIC 값이 각각 0.750∼1.125로서 항생제간의 부분적 상승 또는 상가 작용이 확인되었다. 아목시실린과 클라불란산 합제에 대하여 대상동물에서 효과가 나타나는 양의 2배 용량(아목시실린 + 클라불란산 유효성분 2회/일 투여량기준, 11.76 + 2.94 mg/kg bw/day)으로 하여 비글견에 시험물질을 7일간 경구투여하고, 투여 개시일로부터 14일간 증체량, 사료효율, 임상증상을 관찰한 결과, 특이적인 이상증상이 관찰되지 않았다. E. coli 배양액 10 mL (3 ⨯ 108 Colony Forming Unit(CFU)/mL)을 개의 방광에 1회 접종하여 요도염을 유발한 후 아목시실린 및 클라불란산 합제를 임상권장량(아목시실린 + 클라불란산 유효성분 2회/일 투여량기준, 5.88 + 1.47 mg/kg bw/day)으로 7일간 경구투여하고 치료효과를 관찰한 결과, E. coli에 의해 유발된 체중감소, 혈뇨, 통증, 요도염증 등의 임상증상이 개선됨을 확인할 수 있었다. 즉, E. coli 접종군에서는 체중 및 사료섭취량 감소, 혈뇨 및 뇨 중 대장균 검출, 뇨화학치 변화(뇨비중, 잠혈, 수소이온농도 등), 조직병리학적 소견(요도 상피세포의 과형성 및 염증세포 침윤 등) 등의 임상증상이 뚜렷이 나타났으나, 아목시실린:클라불란산(4:1) 합제 투여군에서는 임상증상의 발현이 현저히 억제되었고 뇨에서의 균검출도 효과적으로 감소하였다. 결론적으로, 아목시실린:클라불란산(4:1) 합제는 개에서 세균성 질병의 원인체인 E. coli에 대하여 아목시실린 및 클라불란산 단독투여에 비하여 항균효과가 증가하였고 복합 투여에 의한 부분적 상승 및 상가 작용이 확인되었으며, 개에서의 요도염에 대한 치료효과가 인정되는 안전한 약물의 조합으로 평가되었다.

This study aimed to confirm the safety and effectiveness of a combination of Amoxicillin trihydrate and Clavulanic acid to treat Escherichia coli (E. coli) infections in dogs by conducting a Minimum Inhibitory Concentration (MIC) assay on E. coli reference strain ATCC 25922 isolated from dogs and three field isolates. The calculation of Fractional Inhibitory Concentration Index (FIC index) was performed to determine the clinical implications of an Amoxicillin trihydrate/clavulanic acid combination. In addition, the treatment effect and safety of an Amoxicillin trihydrate/clavulanic acid combination (in a ratio of 4:1) was found through the injection of E. coli into the bladder of dogs to cause urethritis. The results are as follows. The MIC assay of Amoxicillin trihydrate on the four strains described above revealed that MIC values were below 6.25 µg/mL and 12.5 µg/mL in three field isolates and the reference strain, respectively. Meanwhile, The MIC values of clavulanic acid were 62.5 µg/mL in the four strains, which indicates low effectiveness when treated alone. A MIC assay of the Amoxicillin trihydrate/clavulanic acid combination (4:1) was conducted on the same strains and the resulting value was below 2.5 + 0.625 µg/mL (Amoxicillin trihydrate + clavulanic acid). It was found that the antimicrobial effect of the combination product was 1.25 ∼ 10 times higher than that of Amoxicillin trihydrate alone and 100 ∼ 200 times than that of separate clavulanic acid. The result indicates a significant increase in antimicrobial effect when treated with the combination product. A checkerboard method was used to evaluate interactions between the two drugs at different doses. With a 0.750∼1.125 FIC index value for Amoxicillin trihydrate and clavulanic acid on E. coli, partial synergism or additivity was observed. A combination drugs with a double dose of the effective dose (effective ingredients of Amoxicillin trihydrate + clavulanic acid two times a day, 11.76 + 2.94 mg/kg bw/day) was administered by oral route to beagles for seven days. Ten mL of E. coli culture media (3 ⨯ 108 Colony Forming Unit(CFU)/mL) wasinoculated into the bladder of dogs to induce urethritis. The recommended dose of the combination (effective ingredients of Amoxicillin trihydrate + clavulanic acid two times a day, 5.88 + 1.47 mg/kg bw/day) was then orally administered for a week to identify treatment effects. The combination was found to have efficacy to improve weight loss, hematuria, pain, inflammation of the urethra, and other clinical symptoms that are caused by E. coli. That is, the E. coli inoculated group showed clear clinical symptoms such as feed consumption decrease, hematuria or detection of E. coli in urine, changes in urine chemistry Specific Gravity, Blood, pH, Protein, Urobilinogen, Nitrite, and Leucocytes and histopathological findings (hyperplasia and inflammatory cell infiltration in urethra epithelial cells; hemorrhage and congestion in submuscular or mucosal layers of the bladder; and inflammatory cell infiltration and congestion in the subepithelial tissue of the renal pelvis). However, in the combination-dosed group (Amoxicillin trihydrate/clavulanic acid combination (4:1)), such clinical symptoms were remarkably inhibited, with effectively decreased detection of E. coli in urine. Consequently, targeting E. coli, the main cause of bacterial diseases in dogs, Amoxicillin trihydrate/clavulanic acid combination (4:1) resulted in an increase of antimicrobial and the rapeutic effect compared to the separate dosing of Amoxicillin trihydrate and clavulanic acid in in vitro and dogs without any side-effects. Also, partial synergism or additivity by the combined treatment was observed. Therefore, combined treatment of Amoxicillin trihydrate and clavulanic acid in 4:1 ratio is evaluated to be a safe and effective combination to treat urethritis in dogs.
