Increasing evidences have shown that tumor maintenance and growth are sustained by a minority population of cancer stem cells (CSCs) within the tumor cell mass. CSCs are posited to be responsible for tumor initiation, growth, generation of distant metastasis and relapse after therapy. The expression of CSC markers in paraffin-embedded tissue sections of hepatocellular carcinoma (HCC) was examined and the result was correlated with the clinicopathologic characteristics. Immunohistochemical staining for the markers believed to be expressed in the CSCs - including epithelial cell adhesion molecule (EpCAM), keratin 19 (K19), CD133, and CD56 - was performed in 82 HCC specimens. EpCAM expression was observed in 56% (46/82) of the HCCs, and K19 expression in 6% (5/82). EpCAM expression was significantly associated with elevated serum alpha-fetoprotein level (P=0.001), microvessel invasion of tumor cells (P=0.030), and high histologic grade (P=0.006). In addition, EpCAM expression was associated with K19 expression (P=0.041). The overall survival and relapse-free survival rates in patients with EpCAM-positive HCC were relatively lower than those in patients with EpCAM-negative HCC. However, the difference was not statistically significant. Only two cases of HCCs were positive for CD133 and CD56, respectively. These results suggest that the expression of EpCAM in HCC might be related to the progression of HCC because the positivity of expression was associated with advanced clinicopathologic features.
