배경: 스포츠와 여가활동에 대한 요구가 증가하고 인구가 노령화 되어가면서 건 손상은 주요한 근골격계 질환이 되고 있다. Poldeoyribonucleotide(PDRN)은 디옥시리보뉴클레오티드 중합체로서 아데노신수용체의 A2A 수용체를 활성화하여 조직의 성장과 신생을 활성화한다. 목적: 본 연구에서는 PDRN이 다양한 성장인자의 분비를 촉진시키고 콜라겐 합성을 유도하며 건의 인장강도를 회복시키는 효과가 있음을 밝히고자 하였다. 방법: 7주령의 36 마리의 백서를 임의로 두 군으로 나눈 뒤 아킬레스 건을 절단시킨 뒤 변형 Kessler 방법으로 건봉합 시행하였다. 18 마리의 실험군에는 매일 kg당 8mg의 PDRN을 각각 1주, 2주, 4주간 복강 내 투여하였으며 대조군에는 같은 양의 생리식염수를 투여하였다. 1주, 2주, 4주 후에 건봉합 주변부를 포함하여 조직을 채취하였으며 측경 양각기를 이용하여 채취한 건의 면적을 측정하였으며 기계적 시험을 통해 생물역학적 성질을 측정하였다. 면역 조직화학 염색을 통해 Ⅰ형, Ⅲ형 콜라겐, 형질전환 증식인자β1, 섬유아세포 증식인자, 혈관표피 성장인자의 발현 정도를 확인하였다. 면역 조직화학의 분석을 위해 각 염색 정도를 점수화하여 평가하였으며 Mann-Whitney U test 를 이용하여 통계적 분석을 실시하였다. 결과: 봉합된 건의 면적은 PDRN치료 그룹에서 2주, 4주차에 유의하게 높은 결과를 나타내었으며 (p = 0.008, 0.017) 기계적인 부하에서도 2주, 4주차PDRN치료 그룹에서 더 높은 저항성을 보였다. Background: Tendon injuries are major musculskeletal disorder with a growing demand in sports and recreation and an aging population. Polydeoxyribonucleotide (PDRN), the compound mixture of deoxyribonucleotides polymers, activates the A2A receptor subtype of adenosine receptors and then stimulates tissue growth and neogenesis. Objective: This experimental study confirms that polydeoxyribonucleotide (PDRN), extracted from the sperm of trout bred, could improve secretion of various growth factor, collagen synthesis, and restoration of tensile strength of the tendon in a rat Achilles tendon injury model. Methods: Thirty six male Sprague?Dawley rats aged 7 weeks were divided into two groups. The Achilles tendon was transected and repaired with modified Kessler’s method. In the experimental group (n = 18), the rats received daily intraperitoneal administration of PDRN (8mg/kg/day for 1, 2, 4 weeks). The control groups received same amount intraperitoneal injections of the normal saline. The rats were sacrificed at one, two and four weeks and repair site tissues were harvested. The harvested tissues were evaluated with digital calipers to measure the cross-sectional area and a mechanical testing machine for biomechanical properties. The presence of collagen type Ⅰ, collagen type Ⅲ, transforming growth factor β1, fibroblast growth factor, and vascular endothelial growth factor were confirmed by immunohistochemistry. For immunohistochemical analyses, each immunohistochemical evaluation was scored on a point-based system. Statistical analyses were performed using the Mann-Whitney U test. Results: The cross-sectional area of the tendon was significantly increased at 2 and 4 weeks in PDRN group (p = 0.008, 0.017). Moreover, Tendons in the PDRN group were more resistant to mechanical stress at 2 and 4 weeks compared with that of the control group (p = 0.041, 0.041). The levels of staining of collagen type I in the experimental group were significantly stronger than those in the control group at 2 and 4 weeks (p = 0.026, 0.009) Furthermore, higher expression of FGF, VEGF, and TGF-β1 was detected in the experimental group at 4 weeks compared with that in the control group (p = 0.041, 0.026, 0.041) Conclusion: This study confirms that intraperitoneally injected PDRN can improve tensile strength in rat Achilles tendon healing in rats. Our results represent a new valuable agent in tendon healing. Further studies should be conducted to establish the mechanism of PDRN in different phases during the tendon healing process and to prove safety for application in humans and to establish the optimal doses that should be used.
