The purpose of this study was to evaluate the effects of the core exercise and the neck stabilizing exercise on balance and proprioception in children with cerebral palsy due to premature birth. To identify the effect of exercise, we measured the standing balance, cranio-cervical flexion test(CCFT) and deep neck flexor endurance, head repositioning accuracy(HRA) after doing the neck stabilizing exercise and core exercise(combination of neck stabilizing and trunk stabilizing exercise). 36 subjects were randomly assigned to an experimental group Ⅰ(n=13), an experimental group Ⅱ(n=12), and a control group(n=11). The control group performed conservative physical therapy for 40 minutes. All subjects of the experimental group performed conservative physical therapy for 30 minutes. The experimental group Ⅰ(the combination exercise group) added to the combination of core exercise and neck stabilizing exercise for 10 minutes and the experimental group Ⅱ(the neck stabilizing exercise group) added to the neck stabilizing exercise for 10 minutes. The exercise did not exceed 40 minutes, 3 times a week for 6 weeks. The results of this study were as followed. There were statistically significant changes in the standing balance, the strength and endurance of the neck muscles and HRA. This was true for both experimental groups, with more effective changes seen in the combination exercise group. Changes in medial-lateral speed of center of posture(COP), anterior-posterior distance of COP and velocity moment, which are measures of standing balance, significantly reduced(p<.05). Both experimental group Ⅰand Ⅱ showed significant increase CCFT and deep neck flexor endurance in post-value measurement(p<.05), the experimental group demonstrated significant increase compared with other group in between group comparisons(p<.05). According to the results of the HRA evaluation, HRA decreased after the exercise in both groups, signifying improvement in participants' proprioceptive sense. More specifically, the experimental group Ⅰ's proprioception for the head was effectively enhanced during extension(p<.05). In conclusion, these findings suggested that the neck stabilizing exercise combined with the core exercise could improve the standing balance, muscle strength and endurance, proprioception in children with cerebral palsy. In addition, continuous further studies of the core and the neck stabilizing exercise program and intervention were necessary to improve the balance and proprioception. We hope to develop physical therapy home program for cerebral children that can be easily alone.
