본 연구는 간호 학생의 치료적 의사소통을 위해 표준화 환자를 활용한 학습 시나리오를 개발하고 효과를 평가하기 위해 시행되었고, 학습 시나리오 개발을 위한 방법론적 연구와 효과를 평가하기 위한 비동등성 유사 실험 연구로 설계되었다. 치료적 의사소통을 위한 표준화 환자 학습시나리오의 개발은 ADDIE 모형을 기반으로 문헌고찰을 통해 개발하였다. 개발된 학습 시나리오는 전문가에 의한 내용 타당도 검증과 시범적용을 통해 수정 보완하여 결정하였다. 결정된 학습 시나리오의 적용효과를 평가하기 위한 연구 대상자는 A시 소재 C 대학 간호학과 4학년 재학생을 대상으로 총 20명을 선정하여 실험군과 대조군 각 10명씩 배정하였다. 연구 평가도구는 박향진(2012)이 개발한 간호사의 ‘의사소통 자기효능감’ 측정도구, 권혜숙(2013)의 ‘간호과정 자신감’ 측정도구, 박신영(2012)의 ‘학습 자기효능감’ 측정도구, 박신영(2012)의 ‘학습 만족도’ 측정도구를 사용하였다. 또한 실험군의 ‘치료적 의사소통 지식적용과 기술수행’을 평가하는 도구는 교수, 표준화 환자, 학생용으로 개발하여 사용하였다. 대조군·실험군의 사전·사후조사는 ‘의사소통 자기효능감’, ‘간호과정 자신감’, ‘학습 자기효능감’, ‘학습 만족도’의 평가를 동일하게 실시하였다. 실험군에게는 결정된 학습 시나리오를 적용하는 동안 교수의 ‘치료적 의사소통 지식적용과 기술수행’의 평가가 이루어졌고, 학습 시나리오 적용 직후 표준화 환자와 학생의 ‘치료적 의사소통 지식적용과 기술수행’의 평가가 이루어졌다. 자료 분석은 SPSS 21.0을 활용하여 Non-parametric Wilcoxon Signed-rank test, 평균과 표준편차를 이용하였다. 본 연구의 결과는 다음과 같다. 1. 치료적 의사소통을 위한 표준화 환자 학습 시나리오는 45세 남자 조기 위암환자에게 간호사가 수술 전 간호를 수행할 때 ‘개방성’, ‘경청’, ‘공감’, ‘집중’, ‘격려’, ‘침묵’, ‘요약’, ‘정보제공’의 8가지 치료적 의사소통술을 활용할 수 있도록 개발하였다. 또한 학습목표를 선정하고, 간호과정에 기반한 학습 시나리오 내용을 개발한 후 전문가에 의한 내용 타당도 검증을 통해 간호진단 4개를 도출하는 것에서 2개를 도출하는 것으로 내용을 수정한 후 시범 적용하여 그대로 결정하였다. 2. 학습 시나리오의 효과를 평가한 결과는 다음과 같다. 1) 실험군과 대조군의 ‘의사소통 자기효능감’, ‘간호과정 자신감’, ‘학습 자기효능감’ 그리고 ‘학습 만족도’에 대한 사전 동질성 검증 결과 통계적으로 유의한 차이가 없었다. 2) 치료적 의사소통을 위한 표준화 환자 학습 시나리오를 적용한 실험군이 대조군보다 ‘학습 자기효능감’이 증가하였다(z=-1.99, p=.047). 3) 실험군의 ‘치료적 의사소통 지식적용과 기술수행’에 대한 교수, 표준화 환자, 학생의 평가는 교수 평가가 30점 만점에 평균 27.3점으로 가장 높았다. 또한 교수는 실험군이 ‘격려’(2점 만점에 평균 1.8점), ‘요약’에서 환자에게 질문 기회를 주는 것(2점 만점에 평균 1.8점)의 치료적 의사소통술을 잘 수행한 것으로 평가하였고, 표준화 환자는 실험군이 ‘침묵’(2점 만점에 평균 2점), ‘요약’에서 설명에 대한 환자 이해를 물어 보는것(2점 만점에 평균 2점)의 치료적 의사소통술을 잘 수행한 것으로 평가하였다. 반면에 실험군 스스로는 교수나 표준화 환자의 평가보다 ‘개방성’(2점 만점에 평균 1.9점), ‘경청’(2점 만점에 평균 1.9점), ‘공감’(2점 만점에 평균 1.9점)의 치료적 의사소통술을 잘 수행한 것으로 평가하였으나 ‘집중’(2점 만점에 평균 1.4점), ‘요약’에서 환자에게 질문 기회를 주는 것(2점 만점에 평균 1.1점), ‘정보제공’에서 환자 질문에 알기 쉽게 구체적으로 설명하는 것(2점 만점에 평균 1.4점)의 치료적 의사소통술을 잘 수행하지 못한 것으로 평가하였다. 4) 디브리핑에 대한 내용 분석 결과 실험군은 수술을 앞둔 조기 위암 환자인 표준화환자의 간호문제로 통증관리에 대한 지식부족와 수술 후 식이요법에 대한 지식부족을 정확히 파악하였다. 또한 파악한 간호문제를 해결할 수 있는 간호활동을 수행하였다는 성취감을 가졌다. 또한 간호문제를 파악하고 해결하는 과정에서 정보제공의 치료적 의사소통술을 잘 수행한 것으로 평가하였으나 ‘공감’, ‘요약’, ‘격려’, ‘침묵’의 치료적 의사소통술을 수행하는 것이 어려운 것으로 평가하였다. 치료적 의사소통술을 수행하는 것의 어려움을 극복하기 위해서는 환자중심으로 생각하고 환자와 많은 대화를 나누며 치료적 의사소통술에 대한 훈련이 필요하다고 제시하였다. 결론적으로 간호학생에게 적용한 치료적 의사소통을 위한 표준화 환자 학습 시나리오를 활용한 시뮬레이션 학습은 간호학생의 ‘학습 자기효능감’을 향상시켰다. 또한 간호학생은 간호문제를 파악하고 해결하는 과정에서 정보제공의 치료적 의사소통술을 잘 수행한 것으로 평가하였으나 ‘공감’, ‘요약’, ‘격려’, ‘침묵’의 치료적 의사소통술을 수행하는 것이 어려운 것으로 평가하였다. 따라서 간호학생의 학습 자기효능감을 높이기 위해 시뮬레이션 학습을 적용할 수 있는 구체적인 교수·학습전략이 수립되어야 한다. 또한 간호학생들의 치료적 의사소통술을 증진시키기 위해 반복 훈련의 기회를 제공할 수 있는 방안도 마련되어야 할 것이다.

The purpose of this methodological research study was to develop a standardized patient learning scenario for therapeutic communication for use with nursing students and to evaluate its effect with a nonequivalent quasi-experimental design. A literature review based on the ADDIE(Analysis, Design, Development, Implementation, Evaluation) model was used to develop the standardized-Patient Learning Scenario for therapeutic communication. The developed learning scenario was revised and supplemented through verification of content validity by experts and a pilot study. In order to evaluate the application of the developed learning scenario, 20 senior undergraduate nursing students from an urban university in the eastern part of South Korea were selected as research participants. The participants were divided into two groups: a control group (N=10) and an experimental group (N=10). The evaluation tools utilized in this study were, 'Communication Self-Efficacy' scale for nurses (Park, 2012), 'Nursing Process Confidence' scale (Kwon, 2013), 'Learning Self-Efficacy' scale (Park, 2012), and 'Learning Satisfaction' scale (Park, 2012). For the development of the 'Therapeutic Communication Knowledge Application and Technique' scale, the experimental group was evaluated by professors, standardized-patients, and the nursing students themselves. Pre-test and post-test evaluations were obtained from the scores for 'Communication Self-Efficacy', 'Nursing Process Confidence', 'Learning Self-Efficacy' and 'Learning Satisfaction' scales completed by both the control and experimental groups. Evaluations by the standardized-patients and self-evaluations by the nursing students in the experimental group were performed directly after the scenario was completed. Data were analyzed using Non-parametric Wilcoxon Signed-rank test and SPSS 21.0 was used for average scores and standard deviations. Results of this research are as follows: 1. The Standardized-Patient Learning Scenario for Therapeutic Communication was developed to utilize the 8 therapeutic communication techniques of openness, active listening, empathizing, focusing, encouraging, providing information, using silence, and summarizing by nurses providing pre-operative nursing care to 45 year-old patients with early gastric cancer. The content validity was verified by experts after selecting the learning objectives and developing the nursing curriculum based on content of the learning scenario. Through this validation process, content modifications were made to deduce 2 nursing diagnoses instead of 4 nursing diagnoses. Then, a pilot study was conducted 2. Results of learning scenario effect evaluation are as follows: 1) The pre-homogeneity test of 'Communication Self-efficacy', 'Nursing Process Confidence', 'Learning Self-Efficacy' and 'Learning Satisfaction' indicated that there was no statistical correlation between the control and the experimental group on these variables. 2) The experimental group, the group with which the Standardized-Patient Learning Scenario for Therapeutic Communication was used, showed higher 'Learning Self-Efficacy' (z=-1.99, p=.427) than the control group. 3) From the experimental group's 'Knowledge Application and Technique Use of Therapeutic Communication', evaluations were completed by the professors, standardized-patients, and nursing students. The evaluation by the professors had the highest score at 27.3 out of 30. The evaluations of the professors on the experimental group students also showed high scores for the efficient therapeutic communication techniques of 'encouragement' (score of 1.8 of 2) and 'summarizing- providing opportunities to ask questions' (score of 1.8 out of 2). The standardized patients also evaluated the students as having efficient therapeutic communication techniques in 'using silence' (score of 2 out of 2). However, the students' self-evaluation scores for therapeutic communication techniques were higher for 'openness' (score of 1.9 out of 2), 'active listening' (score of 1.9 out of 2), and 'empathy'(score of 1.9 out of 2), compared to the evaluation scores given by the professors or standardized-patients. However, the nursing students from the experimental group were able to gain a sense of achievement in performing nursing activities through the process of identifying and solving nursing problems. Also, in this process, the students were evaluated as having efficient therapeutic communication techniques in 'providing information' but inefficient techniques in 'empathizing 'summarizing', 'encouraging' and 'using silence'. In order to develop efficient therapeutic communication techniques, it is recommended that the students have opportunities to provide patient-centered care, frequent communication with patients, and use therapeutic communication techniques. In conclusion, nursing students using simulation learning with the Standardized-Patient Learning Scenario for Therapeutic Communication showed improvement in their self- efficacy in learning. Thus, specific teaching and learning strategies using simulation learning and opportunities for repetitive training should be established and provided to enhance the learning self-efficacy and therapeutic communication techniques of nursing students.
