연구에 활용된 초미세먼지 등의 질량농도는 환경부 국립환경과학원에서 운영하고 있는 백령도(인천시 옹진군), 수도권(서울시 은평구), 중부권(대전시 중구), 남부권(광주시 북구) 대기오염집중측정소에서 2011년부터 2013년까지 측정·분석한 일자료와 동일 지역의 국민건강보험심사평가원의 진료비 청구 심사 일자료를 사용하였다.
연구 결과는 다음과 같았다.
첫째, PM2.5 농도는 서울지역에서는 발생 당일 ‘만성 하기도 질환’에, 대전에서는 ‘고혈압 질환’과 ‘상기도의 기타 질환’에, 광주지역에서는 ‘급성상기도 감염’ 백령도지역에서는 ‘허혈성심장질환’과 ‘만성하기도질환’에, 발생 1일 후는 광주지역에서 ‘만성 하기도 질환’ 환자수 발생에 양의 영향을 미치는 것으로 나타났다.
둘째, EC농도는 발생 당일 서울지역에서 ‘고혈압 질환’과 ‘뇌혈관 질환’에 광주에서는 ‘고협압’, ‘허혈성 심장질환’, ‘뇌혈관 질환’, ‘기타 급성 하기도감염’, ‘상기도 기타감염(총 5가지)’에 양의 영향을 미쳤다. 발생 1일 후에는 광주지역에서 ‘기타 급성 하기도감염’에, 발생 2일 후 서울지역에서 ‘동맥, 세동맥 및 모세혈관의 질환’에, 발생 3일 후에는 백령도지역에서 ‘뇌혈관질환’에, 발생 4일 후에는 대전지역에서 호흡기질환인 ‘급성상기도 감염’, ‘기타 급성 하기도 감염’, ‘상기도의 기타 질환’, ‘만성 하기도 질환’에, 발생 5일 후에는 서울지역에서 ‘상기도의 기타 질환’ 환자수 발생에 양의 영향을 미치는 것으로 나타났다.
셋째, OC 농도의 건강 영향은 발생 2일 후 대전지역에서 ‘급성상기도 감염’과 ‘상기도의 기타질환’ 환자수 발생에 양의 영향을 미치는 것으로 나타났다.
또한 4개 지역에서 기온과 습도는 대부분의 순환기계 및 호흡기계 질환 환자수 발생에 음의 영향을 미치는 것으로 나타났는데 이것은 여름철보다 기온과 습도가 낮은 겨울철에 초미세먼지의 농도가 높게 나타나는 계절적 특성이 반영된 결과인 것으로 보인다.
위와 같은 연구결과 초미세먼지와 그중 탄소성분(EC 및 OC)은 순환기계 및 호흡기계의 환자수 발생에 양의 영향을 미침을 확인할 수 있었다. 특히 EC성분는 서울과 광주지역에서 연구대상 질병 10가지 중 5가지에 양의 영향을 미치는 것으로 나타나 EC가 PM2.5 보다 위해성이 크다는 선행연구들의 결과와 유사하게 나타났다. 향후 대기환경 및 국민보건 정책 수립 시에 고려되어야 할 점을 시사해 주는 의미 있는 결과라고 할 수 있다.
다만 이번 연구에서 활용된 초미세먼지 등의 농도 측정위치는 우리나라의 수도권, 중부권, 남부권 및 배경지역의 대푯값으로 광역적 의미를 지니고 있으나 이에 비해 질병자료는 그보다 훨씬 좁은 범위의 행정구역(구단위)단위를 사용하여 그 상관관계 해석에 미흡한 부분이 있다. 또한 초미세먼지(PM2.5) 및 탄소성분(EC 및 OC)의 농도자료가 2011년부터 2013년까지 3년 자료로 충분하지 않았으며 그중에서도 기상 및 측정기기의 상태에 따른 결측이 상당량 존재하여 통계분석 시 제한점이 있었다. 향후에는 충분한 농도자료와 대상지역의 특성을 잘 반영할 수 있는 측정위치 선정 및 그에 상응하는 질병자료 등을 통하여 좀 더 종합적이고 체계적인 연구가 필요할 것이다.

In particular, the significant amount of PM2.5(33.8%, JIN Hyunah, et al., 2011) is generated by mobile sources, and of these black carbon(BC) has been reported to have a higher risk than PM2.5 released from other sources. As a result, further studies are required to investigate the health effects of EC among fine particles(Gan et al., 2011; Laden et al.,2006; Peng et al., 2009).
In this study, we aim to provide theoretical basis for counter-measure plan of air pollution by understanding the Circulatory and Respiratory effects of ultrafine particles in urban areas, especially elemental carbon(EC) and organic carbon(OC) and predicting the future patients.
For mass concentrations of fine dust, especially of EC and OC, we used the data which have been measured and analyzed from 2011 to 2013 by air pollution monitoring stations in Baengnyeongdo Island(Incheon), metropolitan area(Seoul), central region(Daejeon), and southern region(Gwangju) under the National Institute of Environmental Research, and screening data of claim request in the same region by National Health Insurance Review and Assessment Service.
The research results are as follows:
First of all, it was found that on the first day when PM2.5 appeared, its concentrations had impacts on the number of patients suffering from chronic lower respiratory diseases in Seoul, high blood pressure and upper respiratory diseases in Daejeon, acute upper respiratory infection in Gwangju, and ischemic heart diseases and chronic lower respiratory diseases in Baengnyeongdo, while having impacts on the patients suffering from chronic lower respiratory diseases in Gwangju on the 2nd day.
Secondly, EC concentrations had impacts on two diseases(high blood pressure, cerebrovascular disease) in Seoul and five diseases(high blood pressure, ischemic heart disease, cerebrovascular disease, acute lower respiratory infection, upper respiratory diseases, chronic lower respiratory diseases) in Gwangju on the first day. It was also found that the concentrations affected the number of patients with acute lower respiratory infection on the 2nd day in Gwangju, artery, arteriola, capiilary diseases in Seoul on the 3rd day, cerebrovascular diseases in Baengnyeongdo on the 4th day, respiratory diseases in Daejeon on 5th day, and upper respiratory diseases in Seoul on the 6th day.
Thirdly, the health effect of OC concentration affected the number of the patients with acute upper respiratory infection and other upper respiratory disease in Daejeon after two days when OC was first generated.
In addition, temperature and humidity were found to have negative effects on the number of patients with circulatory and respiratory diseases in four regions. This is because of seasonal characteristics that the concentration of fine particle(PM2.5) is higher in winter with lower temperature and humidity compared to summer.
These findings showed that fine particles and carbon compounds including EC and OC had impacts on the number of patients with respiratory and circulatory diseases. In particular, EC concentrations were found to have impacts on five out of ten diseases for research in Seoul and Gwangju, indicating considerations to be made for policy making on air environments and public health.
While measuring sites of fine particle(PM2.5) concentrations used in this study are found to have a wide-area significance, disease data were obtained from local districts, indicating insufficient evidences to interpret the correlations. Also, many limitations existed for statistical analysis, since concentration data of fine particles and carbon compounds had been measured and analyzed only for 3 years with many missing data due to unstable weather and measuring devices. In the future, further studies are needed by better reflecting more concentration data and regional characteristics with more segmented disease data including sex and practice patterns.
