Guidelines provide information on recommendations that aid in decision-making, recommendations that are systematically developed through investigation of scientific evidence. Guidelines for Alzheimer's disease can be classified into diagnosis, pharmacological interventions, and non-pharmacological interventions. Non-pharmacological interventions, which are used by occupational therapists in professional and specialized ways, consist of behavior management, cognitive stimulation, environmental design, multisensory stimulation, physical activities, and caregiver interventions. Since the role of occupational therapy in increasing numbers of Alzheimer's disease patients is expanding, this study aimed to develop an evidence-based occupational therapy guideline for Alzheimer’s disease based on the Occupational Therapy Practice Framework in order for occupational therapists to choose appropriate evaluations and interventions and provide effective, economical, and professional services to patients with Alzheimer's disease and their caregivers. This study developed a guideline through development and adaptation, based on the Guideline for Development of Clinical Practice Guidelines. We searched for occupational therapy guidelines on Alzheimer's disease published in other countries, and selected five guidelines out of 37 identified. We then evaluated them with the Appraisal of Guidelines for Research and Evaluation II (AGREE II) tool. We summarized the contents of the five selected guidelines, and used the summary to formulate the first draft of our guideline, including grades of recommendation. Among 12 items in the evaluation domain, evaluation of depression had a grade of recommendation of A, and among 67 items in the intervention domain, 15 items including behavior management, fall prevention, sleep routines, compensation strategies, home modification, caregiver education, and physical activities had grades of recommendation of A. In order to identify additional evidence on occupational therapy conducted in Korea, we conducted a literature review and found 185 articles. Of these, 58 articles on occupational therapy evaluation and intervention were selected and analyzed. We confirmed the level of evidence of each article, and classified items in the articles into 16 items of occupational therapy evaluation and 20 items of occupational therapy intervention. The final guideline was completed by combining the first draft and the items obtained through additional literature review based on the Occupational Therapy Practice Framework. The items were combined into domains of area of occupation, performance skills, performance patterns, environment, activity demands, and caregivers. In the area of occupation domain, evaluation of and intervention for activities of daily living (ADL) and evaluation of instrumental activities of daily living (IADL) had high grades of recommendation (A). In the performance skill domain, global cognitive function evaluation, attention evaluation, memory evaluation, orientation evaluation, psychosocial evaluation, behavior management, gait evaluation, functional movement evaluation, balance evaluation, and physical activity performance showed high grades of recommendation (A). In the performance pattern domain, sleep routines strategy exhibited a high grade of recommendation (A); in the physical environment domain, grades of recommendation were high (A) in compensational strategy and fall prevention. In the activity demands domain, a high grade of recommendation (A) was observed in implementation of various interventions, while caregiver education and consultation had high grades of recommendation (A) in the caregiver domain. In order to assess the clinical applicability of the completed guideline, we evaluated its appropriateness. When surveys were conducted on 33 specialists in occupational therapy for Alzheimer's disease, all items except driving evaluation and snoezelen had high appropriateness with scores greater than 6. Based on the Occupational Therapy Practice Framework, this study clarified the scope of the activity and the role of occupational therapists in Alzheimer's disease, and suggested grades of recommendation for commonly used evaluation tools and intervention domains. Studies with high levels of evidence have been conducted on various intervention methods, thereby confirming the effectiveness of occupational therapy. Moreover, we confirmed that occupational therapists were using various therapeutic approaches in the management of Alzheimer's disease, and we suggested grades of recommendation in order for effective approaches to be taken in each domain.
