Nurses' Knowledge about the use and management of Patient Controlled Analgesia(PCA) and their Utilization Status Regarding PCA원문보기

Abstract The purpose of this study was to identify nurses’ knowledge about the use and management of Patient-controlled analgesia (PCA) and their utilization status regarding PCA, in order to provide the baseline data for the development of an educational program for nurses. Participants in this study were 182 nurses working in surgical unit from 4 General hospitals of more than 300 beds in Daejeon. Data collection was conducted through the use of self-report questionnaires from November 4th to November 20th 2015. Collected data were analyzed using SPSS 18.0 program for descriptive statistics, t-test, ANOVA. The main results of this study were following; 1. The mean score of nurses’ knowledge about PCA use and management was 14.8 out of 20. There were statistically significant difference noted between the nurses’ knowledge about PCA use and management and their educational background. That is, the knowledge score about PCA use and management of nurses who graduated from the university was higher than those nurses which graduated from the technical colleges (p< .001). 2. Among the nurses' knowledge questionnaire items about PCA use and management, those showing a lower percentage of correct answers were ‘Nurses, family members and care givers can press the PCA button for patients’ (13.7%)’ and ‘About patient education for PCA use, it is effective for nurses to use it after operation in the recovery room or hospital room’ (16.5%), and ‘hypoglycemia or mental disorder patients can use PCA’ (39.0%). 3. 91.2% of subjects said job education about PCA is needed in a clinical setting. 78% of subjects don’t have PCA-related protocols in their hospital or ward, and only 13.2% of nurses were providing PCA education to the patients before surgery. The frequency of PCA-related patient education was 2 times per patient commonly, and the average amount of education time was 5.7 minutes. 87.9% of subjects thought PCA is effective for pain management. The complications of PCA, which often occur in patients, were nausea, vomiting, dizziness, headache, drowsiness, and pruritus. 4. 70.3% of subjects evaluated the patients' vital signs regularly, and 62.1% of them evaluated the level of consciousness regularly. From the above results, it is suggested that the nurses’ knowledge level of PCA use and management was not sufficient to provide safe and effective pain management to the postoperative patients. Therefore, it concluded that there is a need to develop standardized PCA educational programs for nurses to provide safe and effective pain management to the postoperative patients with PCA. Further studies are needed to verify and accumulate empirical evidence of the effects of a PCA education program for nurses.
