Post-tonsillectomy bleeding is one of the most common complication and responsible for the majority of post-operative fatalities. This study evaluated the risk factors of post-tonsillectomy. The medical records of 5,762 tonsillectomized patients were reviewed for the frequency of post-tonsillectomy bleeding who undergone tonsillectomy from January 2005 to July 2013. Of 5,762 patients included in the study, 205 (3.56%) patients with postoperative bleeding were analyzed for age, sex, operators, timing of bleeding and bleeding control methods. Post-tonsillectomy bleeding was common in males with the incidence of 3.99% (P = 0.026), of which the patients with age ≥ 18 y had higher incidence of bleeding with 8.05% (P < 0.001). There was statistical significance associated with operators (P < 0.001) with the incidence of 5.69% in residents, 6.10% in fellows and 2.57% in staffs. In the 205 patients with post-tonsillectomy bleeding, the primary and secondary types accounted for 9% and 91%, respectively and Type I, II and III accounted for 32%, 52% and 16%, respectively. We analyzed the type of bleeding in relation to age, sex and operators. There were statistical significance in control method of bleeding in relations to operators (P = 0.042) and age of the patients (P<0.001). There was no statistical significance between primary and secondary bleeding types according to age and sex of the patients and operators. Bleeding from upper pole were more frequently occurred in patients with primary post-tonsillectomy bleeding (84.2%) than in patients with secondary bleeding (53.8%) (P = 0.014). We conclude that age, sex and operators are associated with post-tonsillectomy bleeding. An awareness of these factors can help identify potential postoperative bleeders.
