현재 전 세계적으로 비타민D 결핍으로 인한 건강 상의 문제가 대두 되고 있고 최근 체내의 많은 세포에서 비타민D 수용체 및 활성화 효소가 발견되면서 비타민D의 근 골격계 외의 역할을 입증하기 위한 연구결과들이 활발히 진행되고 있다. 그 중 대표적으로 비타민D는 당 대사 장애, 당뇨 전 단계와의 관련성뿐만 아니라 심혈관 질환의 위험을 높이는 대사적 위험 인자들과도 관련이 있음을 여러 연구결과로 발표되었다. 그러나 대부분 연구 대상이 성인으로만 한정되어 있고 대사증후군 및 심혈관 질환의 예방적 중재가 필요한 당뇨 전 단계 성인을 대상으로 연구가 제한적이며, 특히 한국 당뇨 전 단계의 성인에서의 관련성을 확인한 연구는 전무한 실정이다. 따라서, 본 연구에서는 당뇨 전 단계의 한국 성인의 혈중 비타민D 상태를 평가하고 이와 관련된 대사적 위험 인자들의 관련성을 확인하고자 하였다. 본 연구는 단면연구이며 공복혈당이 100~125mg/dl에 해당하는 20~65세 성인 60명을 대상으로 약 5개월간 진행되었다. 일반사항조사, 신체계측 및 혈압측정, 식이섭취를 조사하였고 혈액을 분석하였다. 또한 혈중 비타민D 상태를 반영할 수 있는 합성 및 식이 인자를 확인하기 위해 자외선 노출 관련 조사와 비타민D 섭취빈도조사를 함께 진행하였다. 대사증후군은 2009년 AHA, IDF 와 NHLBI의 협의 끝에 제시된 기준을 사용하였고 대상자들은 2011년 IOM에서 발표한 혈중 비타민D 상태 평가 기준에 따라 혈청 25(OH)D 농도가 <12ng/ml은 결핍, 12~19ng/ml은 불충, ≥20ng/ml은 정상으로 세 그룹으로 분류하였다. 연구 대상자의 75%는 비타민D 상태가 결핍 또는 불충분이고 평균 혈청 25(OH)D는 17.3±0.8ng/ml으로 나타났다. 비타민 D 불충분과 정상 그룹과 비교 시 비타민D 결핍그룹은 고중성지방혈증 및 낮은 HDL 콜레스테롤 상태를 동반한 비율이 35.7%으로 더 높았다. (불충분 그룹, 6.5%; 충분그룹 0.0%) 또한 고중성지방혈증 및 낮은 HDL 콜레스테롤 상태를 동반한 대상자의 경우 평균 혈청 25(OH)D 농도가 12.7±0.9ng/ml으로 매우 낮았다. 본 연구에서 혈청 25(OH)D 농도와 혈중 중성지방농도 및 동맥경화지수의 역의 관련성을 확인할 수 있었다. 따라서, 본 연구의 결과에 따라 당뇨 전 단계의 국내 성인의 비타민D상태는 불충분하며 충분한 혈청 25(OH)D 농도는 전 당뇨병 국내 성인의 지질 대사의 긍정적인 역할을 할 가능성을 있을 것으로 사료 된다.

Evidence for the increased risk of MetS and its components in individuals with prediabetes and a low vitamin D status is limited and uncertain. Furthermore, it has not been confirmed whether this possible relationship occurs in the Korean population. The aim of this study was to assess serum vitamin D status and to examine the relationship between serum vitamin D levels and MetS including its components in Korean adults with prediabetes. SUBJECTS/METHODS: This cross-sectional study was conducted among 60 subjects aged 20-65 years. Participants had fasting glucose levels of 100 to 125 mg/dl. A questionnaire was used to assess vitamin D intake, including vitamin D synthesis from sun exposure and dietary intake. Clinical and biochemical data were also collected. The 2009 harmonized definition of MetS was used. Serum vitamin D levels were classified according to criteria from the 2011 Institute of Medicine report. RESULTS: The majority of subjects (75%) had a serum 25(OH)D level < 20 ng/ml, and among them, 30.0% were vitamin D deficienct (< 12 ng/ml). The proportion (42.9%) of subjects having low HDL-cholesterol was the highest among vitamin D deficiency (< 12 ng/ml) group. (12 to < 20 ng/ml: 16.1%, ≥ 20 ng/ml: 6.7%). We observed an inverse relationship between 25(OH)D levels and TG, AI (β= -6.526, SE=2.469; β= -0.020, SE=0.007) after adjusted confounders. CONCLUSION: Korean adults with prediabetes were more likely to have low serum 25(OH)D levels. A sufficient 25(OH)D level may have possible beneficial effects on lipid profile.
