본 연구는 반복 측정을 이용한 비동등성 대조군 전·후 설계로, 연구 대상자는 강원도 W시 상급종합병원 수술실에서 척추마취하에 수술을 받는 정형외과 환자를 근접 모집단으로 대상자 선정기준에 맞는 대상자를 임의 표출하여 음악요법을 받은 실험군1 14명, 귀마개를 적용한 실험군2 14명, 대조군 14명으로 총 42명이었다. 연구도구는 ‘상태불안척도’와 숫자등급척도(Numeric Rating Scale)를 이용한 ‘불안강도’, ‘중재 만족도’이었다. 자료수집 기간은 2016년 3월 14일부터 2016년 5월 30일까지였으며, 수술 전날 사전과 수술 30분 후, 사후에 상태불안을 측정하였고, 척추마취 30분 전, 수술직전, 수술 중, 수술직후, 수술종료 30분 후에 불안강도와 활력징후를 측정하였다. 수술종료 30분 후에는 중재 만족도를 측정하였다. 자료분석은 SPSS 18.0 프로그램을 이용하여 기술통계, χ²검정, Fisher’s exact test, one-way ANOVA, paired t-test, Repeated measure ANOVA를 통해 분석하였다.
본 연구의 가설검증 결과는 다음과 같다.
1) 연구가설 1 : “척추마취 수술 중 음악요법을 받은 실험군1, 귀마개를 적용한 실험군2, 대조군 간의 상태불안 점수는 차이가 있을 것이다.”는 세 집단 간의 상태불안 점수에는 통계적으로 유의한 차이가 없어 가설 1은 기각 되었다.

The purpose of this study is to determine the effect of music and earplug use on anxiety, vital signs, and patient satisfaction in patients who undergo the surgery with spinal anesthesia. A pretest-posttest nonequivalent control group design with repeated measurements was conducted.
A total of 42 patients participated. Fourteen patients randomly assigned to music intervention group, 14 patients to earplug-use intervention group, and 14 patients to control group. For music intervention group and earplug-use intervention group, the intervention during the surgery with spinal anesthesia consisted of protocol with systematic relaxing music and earplug use. State anxiety, anxiety intensity, vital signs, and patient satisfaction were measured, using State-Trait Anxiety Inventory, Anxiety Numeric Rating Scale, Systolic and Diastolic Blood Pressure, Pulse, and Satisfaction Numeric Rating Scale. State anxiety and patient satisfaction were measured at baseline and post-intervention. Anxiety intensity and vital signs were measured five times, which were 30 minutes before the surgery, right before surgery, during surgery, right after surgery, and 30 minutes after surgery. Data were analyzed using descriptive statistics, chi-square test, Fisher’s exact test, one-way ANOVA, paired t-test, and repeated measure ANOVA.
The first finding showed that there was no statistically significant difference of state anxiety among three groups. However, there was statistically significant difference of state anxiety between baseline and post-intervention in both music intervention group and earplug-use intervention group. The second finding showed that the interaction of time and group was statistically significant, indicating anxiety intensity in the groups is changing over time but is changing in different ways. Especially there was statistically significant interaction effect on anxiety intensity between music intervention group and control group. The third finding showed that there was no statistically significant effect of group, and interaction of time and group in systolic/diastolic blood pressure, and pulse, indicating that all of them were decreasing in three groups over time before surgery, and increasing over time after surgery in the same ways. The last finding showed that there was statistically significant difference of patient satisfaction among three groups. The data in this research suggest that the relaxing music and earplug-use during the surgery can be applicable in reducing anxiety of patients who undergo surgery with spinal anesthesia. Furthermore, the results have implication that the use of music rather than earplug can be more effective in reducing anxiety of patients.
