Objectives: Intracerebral hemorrhage (ICH), one of the subtypes of stroke with a severe prognosis, accounts for only about 9% of stroke cases, but has up to 52% mortality rate within 30 days after the hemorrhage attack. Previous studies have shown an association between cold temperatures and ICH; however, there have been insufficient studies for evaluation of association between hot temperatures and ICH. Therefore, we investigated the effect of low and high temperature with duration of cold spells and heatwaves on the hospital admissions for ICH in South Korea. Methods: We used the daily hospitalization data of ICH patients aged 20 and over for visits from January 1, 2004 to December 31, 2012, provided by the Korea National Health Insurance Service. The daily meteorological and air pollution data for the 9 years were provided by the Korea Meteorological Administration and the Korea National Institute of Environmental Research, respectively. These data were analyzed by the generalized additive model (GAM) analysis to estimate the association between the risk of ICH and temperature according to gender, age group, urban or rural residence and socioeconomic level with controlling for meteorological and air pollution factors. Results: We found a short-term effect of cold and hot temperature in increasing the risk of ICH. The threshold temperature in the winter season was -12.5℃ as the minimum temperature, and in summer season, it was 27.5℃ as the mean temperature. The risk of ICH was increased by 3% (RR, 1.03; 95% CI, 1.00-1.05) for every 1℃ decrease in minimum temperature below -12.5℃ in winter, and by 6% (RR, 1.06; 95% CI, 1.04-1.08) for every 1℃ increase in mean temperature above 27.5℃ in summer. We found the risk of ICH associated with temperature was higher in urban than rural areas, and in people on Medicaid rather than as National Health Insurance Subscribers. In addition, one day increase in duration of cold spell and heat wave was associated with an increase of the daily number of hospital admissions for ICH by 5% (RR: 1.05, 95% CI: 1.04-1.06) and 1% (RR: 1.01, 95% CI: 1.00-1.02), respectively. Conclusion: The association between temperature changes and ICH was found not only in the winter but also in the summer season. Particularly those of low socioeconomic status or those in urban areas showed a higher risk of ICH associated with both low and high temperature. These results suggested that hot temperatures also had an adverse effect on ICH events, and thus public health efforts should be emphasized not only in winter but also summer season for precautions against ICH, particularly for the vulnerable groups.
