This study was to investigate the effect on improving cardiopulmonary function of chronic stroke conducting treadmill training of hemiplegic patient caused by stroke underwater and ground. I surveyed 30 of chronic stroke and divided them into 14 of underwater treadmill trainers and 13 of ground treadmill trainers except 3 of dropouts. I investigated Heart Rate(HR), Ratings of Perceived Exertion(RPE) and 6 min walk test before and after training to check Forced Vital Capacity(FVC), Forced expired volume in one second(FEV1) and Peak Expiratory Flow(PEF) as a pulmonary function test. As a result of pulmonary function test, Forced Vital Capacity(FVC), Forced Expired Volume in one second(FEV1) and Peak Expiratory Flow(PEF) showed a significant increasing(p.05). As a result of cardiorespiratory endurance test, I found a significant enhancement(p<.05) in Heart Rate(HR), 6 min walk test but no difference between each training group like a pulmonary function test. In case of Ratings of Perceived Exertion(RPE), I found a significant difference only in underwater treadmill training group. I confirmed that underwater treadmill and ground treadmill training effect on cardiopulmonary function of stroke from this study and I considered that both underwater treadmill and ground treadmill training is recommended in Cardiopulmonary Physical Therapy even though there wasn’t a siginificant difference between each group.
