궤양성 대장염(ulcerative colitis)는 면역반응 조절이상을 동반한 다인성 질병이다. 장 상피 장벽(intestinal epithelial barrier)의 붕괴와 박테리아의 침투로 인한 염증이 일반적으로 나타나는 대장염 증상이다. 초기 연구들은 장벽 완전성(intestinal barrier integrity)에 중점을 두었지만, 질병 진행과정 중의 장벽 기능장애 및 밀착연접(tight junction) 단백질 조절의 근원적인 기전은 아직 밝혀지지 않았다. Phospholipase D2 (PLD2)의 활성 증가가 대장염 동물모델의 질병 중증도와 연관한다는 것이 보고되었다. 그러나 장 상피 장벽의 유지 및 궤양성 대장염에서 PLD2의 구체적인 역할은 여전히 불분명하다. 이 연구를 통해 대장염 발병과정에서 상피세포 PLD2의 역할을 분석하고자 하였다. 대장염에서 PLD2의 구체적 역할을 이해하기 위해 상피세포 특이적 Pld2 유전자 결손 생쥐 (intestinal-specific Pld2 knock-out mice, Pld2 IEC-KO)를 사용하였다. 상피세포 특이적 Pld2 유전자 결손은 DSS (dextran sulfate sodium) 또는 DNBS (2,4-Dinitrobenzenesulfonic acid) 에 의해 유도된 생쥐의 대장염 발생을 억제하였다. Pld2 IEC-KO 생쥐는 장 투과성(intestinal permeability) 감소와 함께 밀착연접 단백질인 occludin의 양이 유의미하게 증가하였으며, 이는 장벽 완전성 향상을 의미한다. 흥미롭게도 DSS 투여는 대장 상피세포의 PLD2 발현 증가와 occludin 발현 감소를 유도하였다. PLD2에 의한 occludin 조절 기전 분석 결과는 PLD2가 occludin의 전사(transcription) 및 번역(translation) 과정과는 무관함을 보여준다. 반면, PLD2는 c-Src kinase 의존적 경로를 통해 occludin의 인산화(phosphorylation)와 그로 인한 proteasomal degradation을 유도하였다. 또한 PLD2 억제제(inhibitor) 투여는 생쥐의 DSS 유도 대장염 발생을 억제하였다. 더 나아가 DSS에 의한 PLD2 조절 기전 연구를 수행하여 NFkB가 DSS에 의한 PLD2 증가를 매개하는 전사인자(upstream factor) 임을 입증하였다. 이 연구를 통해 PLD2가 상피세포 연접의 완전성(integrity of epithelial tight junction)과 occludin turn over에 중추적 역할을 함으로써 대장염 발병과정에 관여한다는 것을 입증하였다. 유전적, 약학적 PLD2 억제는 occludin의 양을 회복시켰으며, 생쥐의 대장염의 증상을 완화하고 생존률을 개선하기 충분하였다. 이 연구는 PLD2의 새로운 생리/병리학적 역할을 확인하고, PLD2 억제제가 궤양성 대장염 치료 시 사용되는 전통적 면역 억제제의 병용투여 약물로서의 잠재적 가능성을 제시하였다는 점에서 큰 의의가 있다.

Disruptions to the intestinal epithelial barrier and translocation of bacteria resulting in inflammation are common in colitis. Earlier studies have focused on the integrity of intestinal barrier, but the mechanism of underlying epithelial barrier dysfunction or regulation of tight junction proteins during disease progression have not been elucidated. An increase in phospholipase D2 (PLD2) activity was reported to be associated with disease severity in colitis animal models. However, its specific role in the maintenance of intestinal barrier integrity and in ulcerative colitis remains elusive. In this present study, I analyzed the role of epithelial PLD2 in the pathogenesis of colitis. I generated intestinal-specific Pld2 knock out (Pld2 IEC-KO) mice to understand the specific role played by epithelial PLD2 in colitis. Here I report that Pld2 IEC-KO confers protection against Dextran Sodium Sulfate (DSS) and Di-nitro Benzene Sulphonic acid (DNBS) -induced colitis in mice. Intestinal permeability was low in Pld2 IEC-KO mice with a significant increase in occludin levels, suggesting higher barrier integrity. Interestingly, DSS treatment induces PLD2 expression and down-regulates occludin in colon epithelial cells. Analysis of the mechanism of PLD2 mediated occludin regulation revealed that PLD2 is not involved in transcriptional or translational level regulation of occludin. However, PLD2 mediates occludin phosphorylation and induces its proteasomal degradation in a c-Src kinase-dependent pathway. Additionally, I have shown that PLD2 inhibitor treatment could rescue mice from DSS induced colitis. I also investigated the mechanism of PLD2 regulation by DSS, and demonstrate that NF-κB could be an upstream factor for DSS induced PLD2 up-regulation. In conclusion, this study demonstrates that PLD2 is pivotal in the regulation of the integrity of epithelial tight junction, and occludin turn over, thereby involving in the pathogenesis of colitis. In our study, I show that genetic or pharmacological PLD2 inhibition restore occludin level and it is sufficient enough to alleviate the symptoms of colitis and improve the survival rate of the mice. I thus highlight a novel patho-physiological role for PLD2, and also suggest that, PLD2 inhibitor could be a potential therapeutic strategy when used in combination with conventional immuno-suppressant therapies in the treatment of ulcerative colitis.
