When child patients are admitted to the pediatric intensive care unit, formation of a partnership based on cooperation is essential between mothers who stay with their child and nurses who are in charge of primary care of children in the pediatric intensive care unit. However, the perception of this partnership can differ depending on previous experiences and the current situation, and needs to be further explored. Meanwhile, previous studies have limitations, as they mainly focused on the neonatal intensive care unit, general wards, and the local community, and investigated either parents or nurses.
Hence, this study was conducted to examine in depth the perceptions of the partnership by mothers of hospitalized children and the nurses in a pediatric intensive care unit and the difference between these perceptions using a qualitative study method through semi-structured and open-ended interviews.
Data were collected between January 2 and March 25, 2017, and the study participants included 7 mothers of child patients who had been admitted to the pediatric intensive care unit for longer than 7 days and 12 nurses who had been working in the pediatric intensive care unit for more than 2 years each. Data were collected through semi-structured one-on-one in depth interviews and the duration of interviews was 60 to 90 minutes. The collected qualitative data were analyzed using a classic content analysis method by Morse and Field (1995).
The results of this research can be summarized, 5 common themes were derived from perceptions of the partnership by mothers of hospitalized children in a pediatric intensive care unit: expectation of trust, sharing and communication, participation in care, equality in relationship, and coordination of opinion. Meanwhile, despite the common themes, there were the following differences in the perceptions of mothers and nurses. The categories of the perception of the partnership by mothers included ‘efforts to trust the nurses’, ‘trivial sharing and communication with brave nurses’, ‘common nursing role for my child’, ‘making the position equal’, and ‘continuous coordination of opinion to work together’. On the other hand, the categories of the perception of partnership by nurses included ‘longing for unquestioning trust from parents’, ‘careful approach for communication’, ‘acceptance of limited nurse participation according to the nurse’s judgment’, ‘impartial exchange of information’, and ‘coordination of opinion led by the medical team for the participation of nursing’.
The significance of our study results is as follows. This is the first qualitative study attempted in Korea to investigate the perception of partnership by mothers of hospitalized children and the nurses in a pediatric intensive care unit. In addition, it is also worth noting that the study was conducted in the pediatric intensive care unit, where the severity of disease is high, recurrent hospitalization of chronic illness children and the care is principally by nurses and that it studied the perceptions of both mothers and nurses.
Furthermore, as the severity of disease is high in the pediatric intensive care unit, we found that the partnership is formed in the process of death. This contradicted previous study results claiming that the formation of a partnership is needed for physical recovery of child patients. Therefore, our study uniquely revealed that the formation of a partnership is possible between mothers and nurses even without physical recovery of child patients. In other words, it is important to note that our study attempted to comprehensively understand the partnership in the pediatric intensive care unit.
Moreover, the results of this research will contribute to the development of theories related to partnership in the pediatric intensive care unit and provide a foundation for intervention programs that can enhance the partnership.
