Possible routes are sexual contact, blood transfusion, reuse of contaminated syringes, the use of non-sterilized needles, and piercing. After the virus invades to blood with those routes, they mainly inhabits in liver cells. Our body immune system responses in order to eliminate those viruses. And that causes liver cells to be destroyed, ending up having inflammation. Mostly, no symptoms are shown in the early stage of hepatitis C and chronic infections in adults is 75 % or more, unlike the B-type hepatitis. Acute hepatitis C is a case that infection disappears within 6 months due to body immune system responding to it after the infection. The main symptoms are jaundice, fatigue, or nausea. Chronic hepatitis C is a disease that the body immune system is not able to completely remove the virus, causing the inflammation to last longer that six months. Over the years, 5-20 % of them proceeds with cirrhosis and 1-5 % of which progress to liver cancer. The main symptoms are fatigue, upper right abdominal pain, nausea and loss of appetite. Diagnosis of hepatitis C is examined by blood test for detecting of antibodies or RNA of hepatitis C virus. Genotype of HCV(1a, 1b, 2, 3, 4, 5, 6) is detected by examining the primary site of the 5 UTR region and NS5B with real-time PCR method. In the case with a patient diagnosed as hepatitis but not actually infected with HCV, the examination for determining other potential hepatitis virus(type A or B) should be performed. The diagnosis of hepatitis B is conducted by DNA testing, which is different from that of hepatitis C. As direct-acting antiviral agents (DAA) is developed, the paradigm is changing so dramatically that the standard treatment injecting combined peg-interferon α and ribavirin is declining in other countries including South Korea. DAA is directly activated on a specific part of the non-structural HCV proteins blocking the proliferation, which results in therapeutic effect. And this is classified as NS3/4A protease inhibitor, NS5A inhibitors, NS5B polymerase inhibitor depending on the action mechanism and its target site. As drugs are characterized with both advantages and disadvantages, more than two DAA whose activating targets are different should be prescribed being combined. In the group of which patients are difficult to be treated, additional administering of ribavirin is conducted or the period without having ribavirin is extended to 24 weeks showing sustained virological response (SVR). This review deals with currently available medication and treatment for genotype 1 chronic hepatitis C and the paradigm shift in the future, focusing on the guidelines for chronic hepatitis C published and revised by Korean Association for the Study of the Liver.
