본 연구는 30대 직장인 요통환자 12명을 대상으로 기능교정 치료를 실시하여 8주 후에 족저압 균형 및 골반변위 중 골반경사(Pelvic Tilt), 골반회전(Pelvic Rotation)의 변화, 척추변위의 요추전만각(Lumbar Lordotic Angle)과 천골각(Ferguson's Angle)의 변화에 미치는 효과를 알아보고자 실시하였으며, 이에 대한 분석결과는 다음과 같다. 첫 번째로 참가자들의 기능교정에 대한 족저압의 평균과 표준 편차를 살펴본 결과, 기능교정 전에는 족저압 평균이 좌측 53.67(σ=4.51), 우측 46.32(σ=4.51), 앞쪽 67.74(σ=12.54), 뒤쪽 32.25(σ=12.54)이였던 측정값이 치료 8주 후 족저압은 좌측 50.50(σ=0.83), 우측 49.49(σ=0.83), 앞쪽 56.78(σ=6.70), 뒤쪽 43.21(σ=6.70)로 측정되었다. 족저압 균형에 사전․사후 차이를 알아보기 위하여 대응표본 t-test를 실시한 결과에서는 기능교정 4주 후 좌‧우 족저압과 기능교정 8주 후 좌‧우, 앞‧뒤 족저압의 사전과 사후 변위는 유의미한 차이가 있는 것으로 나타났다. 두 번째로 참가자들의 기능교정에 대한 골반변위의 평균과 표준 편차를 살펴본 결과, 골반 경사의 경우 기능교정 전에는 골반경사 평균이 좌측 222.53(σ=10.60), 우측 221.86(σ=10.42)이였던 측정값이 치료 후에는 좌측 222.13(σ=11.28), 우측 222.48(σ=10.65)로 측정되었다. 골반 회전에 대한 평균과 표준 편차를 살펴본 결과, 기능교정 전에는 골반회전 평균이 6.41(σ=3.86)이였던 측정값이 치료 후에는 3.38(σ=3.14)로 측정되었다. 골반변위의 사전․사후 차이를 분석하기 위하여 대응표본 t-test를 실시한 결과 골반 변위 중 좌측 골반경사의 사전․사후 통계치(t=0.264, p=0.797)와 우측 골반경사의 사전, 사후 통계치(t=-7.68, p=0.459)에서는 유의미한 차이가 없었으나, 골반회전에 대한 사전, 사후 통계치(t=3.611, p=0.004)는 통계적으로 유의미한 차이가 나타났다. 세 번째로 참가자들의 기능교정에 대한 척추변위의 평균과 표준 편차를 살펴본 결과, 천골각의 경우 기능교정 전에는 평균이 37.83(σ=5.11)이였던 측정값이 치료 후에는 40.50(σ=4.34)으로 측정되었다. 요추전만각의 경우 기능교정 전에는 평균이 37.75(σ =7.88)이였던 측정값이 치료 후에는 39.5(σ=5.22)으로 측정되었다. 척추 변위의 사전․사후 차이를 분석하기 위하여 대응표본 t-test를 실시한 결과 척추변위 중 요추전만각의 사전, 사후 통계치(t=-0.922, p=0.377)에서는 유의미한 차이가 없었으나, 천골각에 대한 사전, 사후 통계치(t=-4.257, p=0.001)에서는 유의미한 차이가 있었다. 네 번째로 연구 참가자들의 기능교정 전, 후에 대한 골반변위 및 척추변위와 족저압 비율과의 관계정도를 살펴보기 위하여 Pearson 상관분석을 실시하였다. 기능 교정 전 골반변위의 측정항목인 좌측골반경사, 우측골반경사, 골반회전과 족저압 비율 좌․우, 앞․뒤의 상관관계는 통계적으로 유의하지 않음을 확인할 수 있었으며, 척추변위의 측정항목에서 천골각은 족저압의 좌․우, 앞․뒤의 비율과 유의하지 않게 나왔으나, 요추전만각과 좌․우, 앞․뒤의 족저압 비율과는 유의미한 관계를 가지고 있는 것으로 확인되었다. 연구 참가자들의 기능교정 후 골반변위 중 좌측골반경사는 족저압 좌․우 비율과 유의미한 관계를 나타냈으며, 족저압 앞․뒤 비율과도 유의미한 관계를 나타냈으며, 우측 골반경사는 앞쪽 족저압 비율만 유의미한 상관관계를 나타냈다. 골반변위 중 골반회전은 족저압 좌․우 비율과 족저압 앞․뒤 비율 모두에서 유의미한 관계를 나타냈다. 척추변위의 측정항목에서 천골각은 족저압 좌․우 비율과 족저압 앞․뒤 비율 모두에서 유의미한 관계를 나타냈으며, 요추전만각은 족저압 좌․우 비율에서만 통계적으로 유의하게 나타났다. 이러한 결과는 기능교정 치료가 골반변위, 척추변위에 전반적으로 유의미한 영향을 미치고 있으며, 체간과 하지의 균형에 긍정적인 영향을 주어 족저압의 차이가 개선되어 신체 안정성이 향상되는데 효과가 있음을 확인한 결과라고 할 수 있다. 본 연구의 결과는 향후 요통환자의 치료에 긍정적인 기초 자료를 제공할 것으로 기대한다.

After 8 weeks of functional orthodontic treatment on the subjects of twelve worker patients with back pain in their 30s, this study attempted to investigate the effect of pelvic tilt, pelvic rotation, lumbar lordotic angle and ferguson's angle on pelvic floor balance and pelvic displacement, the following results of analysis were obtained. First, the average and standard deviation of the foot pressure for participants' functional correction, before the functional correction, the mean values of the low pressure averages which had been left 53.67 (σ=4.51), right 46.32 (σ=4.51), front 67.74 (σ=12.54) and back 32.25 (σ=12.54) were measured as 50.50 on the left (σ=0.83), 49.49 on the right (σ=0.83), 56.78 on the front (σ=6.70) and 43.21 on the back (σ=6.70) after eight weeks of treatment. To determine the pre- and post-difference in the foot pressure balance, the results of the corresponding sample t-test showed statistically significant differences between the pre- and post- displacements of the left and right and the anterior and posterior low pressure after functional correction. Second, from the results of examining the mean and standard deviation of the pelvic displacement for the functional correction of the participants, in the case of pelvic tilt, the mean values of pelvic tilt that had been 222.53 (σ=10.60) in the left and 221.86 (σ=10.42) in the right, and 222.13 (σ=11.28) were measured to be left 222.13 (σ=11.28), right 222.48 (σ=10.65). The mean and standard deviation of the pelvic rotation were measured as 6.41 (σ=3.86) and after pelvic rotation it was 3.38 measured as (σ=3.14). From the results of performing the corresponding sample t-test to analyze the pre- and post-difference of pelvic displacement, among the pelvic displacements, there were no significant differences between the pre- and post-mortem statistics of the left pelvic tilt (t=0.264, p=0.797) and the pre- and post-mortem statistics of the right pelvic tilt (t=-7.68, p=0.459). From the results of conducting the corresponding sample t-test to analyze the pre-post-difference of spinal displacement, there was no significant difference in the pre- and post-statistics of the lumbar lordotic angle (t=-0.922, p=0.377) among the spinal displacements. However, there was a significant difference in the pre- and post-statistics (t=-4.257, p=0.001) for ferguson's angle. Fourth, Pearson correlation analysis was performed to investigate the relationship between pelvic displacement and spinal displacement and foot pressure ratio before and after functional correction of study participants. The correlation between left pelvic tilt, right pelvic tilt, pelvic rotation and left to right ratio of the pelvic displacement before and after correction of function was not statistically significant. In the measurement of spinal displacement, the ferguson's angle was not significantly different from the ratio of left and right, and that of front and back. However, it was found that the lumbar lordotic angle had a significant relationship with left-right, front-back ratio. The left pelvic tilt of the pelvic displacement after functional correction of the study participants showed a significant relationship with the low pressure left-right ratio. There was also a significant relationship between the ratio of front and back pressure. The right pelvic tilt showed a significant correlation only with the front low pressure ratio. Pelvic rotation during the pelvic displacement showed a significant relationship in both low to high left and right ratio and front and back ratio. In the measurement of spinal displacement, the ferguson's angle showed a significant relationship in both the left and right ratio and the front and back ratio. The lumbar lordotic angle was statistically significant only in the low pressure left-right ratio. These results indicate that functional orthodontic treatment has an overall effect on pelvic displacement and spinal displacement. The results of this study confirm that there was the effect of improving the body stability by improving the difference of low pressure by positively affecting the balance of trunk and leg. The results of this study are expected to provide positive basic data for future treatment of low back pain patients.
