Myeloid-derived suppressor cells (MDSC) are heterogeneous populations of immature myeloid cells with various myeloid immunophenotypes. Two major subpopulations of MDSCs are granulocytic MDSC (gMDSC) and monocytic MDSC (mMDSC). They promote T cell and natural killer cell dysfunction, and also induce regulatory T cell expansion. In recent years, MDSCs have been extensively studied about their role in suppressing immune responses in malignancies including hematologic malignancies, but most studies provide limited information because of small number of the cases and using patients’ peripheral blood (PB) samples only. The purpose of this study is to investigate the association of gMDSCs and mMDSCs with clinical and laboratory findings in acute myeloid leukemia (AML) and acute B lymphoblastic leukemia (B-ALL), and to clarify correlation between measured MDSCs and prognosis in both diseases. We enrolled 103 AML and 30 adult and 22 pediatric B-ALL patients who were newly diagnosed from September 2013 to September 2014 in Asan Medical Center. We measured MDSC in their PB, bone marrow (BM) samples at diagnosis (Dx) and complete remission (CR). As negative controls (NC), we tested both PB and BM samples of 40 patients who underwent staging work-up of lymphoma or other BM work-up which showed normocellular marrow without BM involvement. For healthy controls (HC), PB samples of 40 healthy donors were used. We measured HLA-DRlow/-CD11b+CD33+CD15+CD14- cells for gMDSC and HLA-DRlow/-CD11b+CD14+ cells for mMDSC. We compared proportion of gMDSCs, mMDSCs in both of PB, BM samples and additionally compared absolute number of MDSCs in PB. As a result, in AML, BM-gMDSC% (median: 2.34%, range: 0.05-40.90), PB-gMDSC% (6.51%, 0.11-78.36), and PB-gMDSC number (No.)/ul (453, 0-24619) at Dx are significantly lower than those of CR (BM-gMDSC%: 41.40%, 0.52-66.40; PB-gMDSC%: 42.35%, 12.32-74.72; PB-gMDSC No./ul: 2146, 160-8564) and control (BM-gMDSC%: 40.25%, 3.60-54.60; PB-gMDSC% of NC: 25.60%, 4.80-52.00; PB-gMDSC% of HC: 18.10%, 5.30-56.90; PB-gMDSC No./ul of NC: 1413, 210-6706; PB-gMDSC No./ul of HC: 1209, 228-5519) groups. We found significant negative correlation of gMDSC with blast proportion among nucleated cells in BM and PB at Dx of AML. As we divided AML patient group into acute and non-acute groups, non-acute group shows tendency to have higher levels of BM-gMDSC%, PB-gMDSC%, and PB-gMDSC No./ul than acute group at the time of Dx. However, BM-mMDSC% (0.37%, 0.01-11.42), PB-mMDSC% (0.64%, 0.02-27.72), and PB-mMDSC No./ul (42; 1-8206) at Dx are higher than those of control group (BM-mMDSC%: 0.25%, 0.00-3.40; PB-mMDSC% of NC: 0.20%, 0.00-2.20; PB-mMDSC% of HC: 0.30%, 0.10-1.40; PB-mMDSC No./ul of NC: 9, 0-290). Additionally, we recalculated BM-gMDSC% and BM-mMDSC% among nucleated cells except the blasts in BM at Dx, and observed significantly increased BM-mMDSC% at Dx different from previous results. In B-ALL, BM-gMDSC% (adult: 2.23%, 0.14-32.58; pediatric: 4.08%, 0.13-20.52), and PB-gMDSC% (adult: 10.09%, 0.16-35.57; pediatric: 7.97%, 0.04-23.33) at Dx are significantly lower than those of CR (adult BM-gMDSC%: 45.58%, 2.79-71.47; adult PB-gMDSC%: 49.40%, 30.20-78.72; pediatric BM-gMDSC%: 24.53%, 1.71-46.98; pediatric PB-gMDSC%: 28.78%, 14.20-57.71) and control (BM-gMDSC%: 40.25%, 3.60-54.60; PB-gMDSC% of NC: 25.60%, 4.80-52.00; PB-gMDSC% of HC: 18.10%, 5.30-56.90) groups. BM-mMDSC% (adult: 0.09%, 0.00-0.95; pediatric: 0.10%, 0.01-2.11), and PB-mMDSC% (adult: 0.19%, 0.01-2.26; pediatric: 0.30%, 0.01-13.85) at Dx are lower than or statistically same with those in CR group (adult BM-mMDSC%: 0.78%, 0.23-3.86; adult PB-mMDSC%: 1.35%, 0.38-9.47; pediatric BM-mMDSC%: 0.31%, 0.12-0.94; pediatric PB-mMDSC%: 0.37%, 0.00-2.07) and control group (BM-mMDSC%: 0.25%, 0.00-3.40; PB-mMDSC% of NC: 0.20%, 0.00-2.20; PB-mMDSC% of HC: 0.30%, 0.10-1.40). However, like in AML, PB-mMDSC No./ul (adult: 28, 1-423; pediatric: 59, 0-876) at Dx is significantly higher than that of NC group (9, 0-290). PB-gMDSC No./ul at CR of 2500/ul or above group shows higher relapse rate (P=0.103) and mortality rate (P<0.05) with shorter RFS (P<0.05) and OS (P<0.05) than PB-gMDSC No./ul at CR of under 2500/ul group. Especially, increased PB-gMDSC No./ul at CR in adult B-ALL significantly indicates poor prognosis. The above findings show a possibility that they could be used as a prognostic marker in the future. The poor prognostic chromosomal abnormality group of adult B-ALL patients shows significantly increased PB-mMDSC No./ul at Dx. Also, as PB-mMDSC No./ul of chemotherapy-treated adult B-ALL patients at Dx is higher in non-survivor group than in survivor group, it may be associated with prognosis.
Myeloid-derived suppressor cells (MDSC) are heterogeneous populations of immature myeloid cells with various myeloid immunophenotypes. Two major subpopulations of MDSCs are granulocytic MDSC (gMDSC) and monocytic MDSC (mMDSC). They promote T cell and natural killer cell dysfunction, and also induce regulatory T cell expansion. In recent years, MDSCs have been extensively studied about their role in suppressing immune responses in malignancies including hematologic malignancies, but most studies provide limited information because of small number of the cases and using patients’ peripheral blood (PB) samples only. The purpose of this study is to investigate the association of gMDSCs and mMDSCs with clinical and laboratory findings in acute myeloid leukemia (AML) and acute B lymphoblastic leukemia (B-ALL), and to clarify correlation between measured MDSCs and prognosis in both diseases. We enrolled 103 AML and 30 adult and 22 pediatric B-ALL patients who were newly diagnosed from September 2013 to September 2014 in Asan Medical Center. We measured MDSC in their PB, bone marrow (BM) samples at diagnosis (Dx) and complete remission (CR). As negative controls (NC), we tested both PB and BM samples of 40 patients who underwent staging work-up of lymphoma or other BM work-up which showed normocellular marrow without BM involvement. For healthy controls (HC), PB samples of 40 healthy donors were used. We measured HLA-DRlow/-CD11b+CD33+CD15+CD14- cells for gMDSC and HLA-DRlow/-CD11b+CD14+ cells for mMDSC. We compared proportion of gMDSCs, mMDSCs in both of PB, BM samples and additionally compared absolute number of MDSCs in PB. As a result, in AML, BM-gMDSC% (median: 2.34%, range: 0.05-40.90), PB-gMDSC% (6.51%, 0.11-78.36), and PB-gMDSC number (No.)/ul (453, 0-24619) at Dx are significantly lower than those of CR (BM-gMDSC%: 41.40%, 0.52-66.40; PB-gMDSC%: 42.35%, 12.32-74.72; PB-gMDSC No./ul: 2146, 160-8564) and control (BM-gMDSC%: 40.25%, 3.60-54.60; PB-gMDSC% of NC: 25.60%, 4.80-52.00; PB-gMDSC% of HC: 18.10%, 5.30-56.90; PB-gMDSC No./ul of NC: 1413, 210-6706; PB-gMDSC No./ul of HC: 1209, 228-5519) groups. We found significant negative correlation of gMDSC with blast proportion among nucleated cells in BM and PB at Dx of AML. As we divided AML patient group into acute and non-acute groups, non-acute group shows tendency to have higher levels of BM-gMDSC%, PB-gMDSC%, and PB-gMDSC No./ul than acute group at the time of Dx. However, BM-mMDSC% (0.37%, 0.01-11.42), PB-mMDSC% (0.64%, 0.02-27.72), and PB-mMDSC No./ul (42; 1-8206) at Dx are higher than those of control group (BM-mMDSC%: 0.25%, 0.00-3.40; PB-mMDSC% of NC: 0.20%, 0.00-2.20; PB-mMDSC% of HC: 0.30%, 0.10-1.40; PB-mMDSC No./ul of NC: 9, 0-290). Additionally, we recalculated BM-gMDSC% and BM-mMDSC% among nucleated cells except the blasts in BM at Dx, and observed significantly increased BM-mMDSC% at Dx different from previous results.
