본 연구는 고관절 골절로 인해 수술을 받은 노인 대상자의 보행시기 및 양상을 파악하고 대상자의 개인적 요인 및 사회적 지지, 낙상에 대한 두려움 등의 인지·정서요인과 조기보행의 관계를 확인한 횡단적 서술적 조사연구이다. 대상자는 서울 소재 상급종합병원에 입원한 고관절 골절 환자 중 수술적 치료를 받은 환자 67명으로, 2017년 2월부터 5월까지 Pender(2006)의 건강증진모형을 기반으로 한 구조화된 설문지와 수술 후 1일째 보행일지를 제공하고 수술 후 보행을 시작한 시간과 매일의 보행거리를 작성하게 하여 자료 수집을 시행하였다. 수집된 자료는 SPSS 24.0 프로그램을 이용하여 실수, 백분율, 평균, 표준편차, t-test, one-way ANOVA, Chi-square test, Fisher’s exact test, Pearson’s correlation으로 분석하였다. 본 연구의 결과는 다음과 같다 1. 고관절 골절 수술 후 보행을 시작하는 시간은 평균 44.58±35.4시간 이후였으며, 수술 후 48시간내에 조기보행을 하는 대상자는 41.8%였다. 수술 후 보행거리(meter)는 수술 후 1일째부터 5일째까지 점차 증가하는 양상을 보였으며, 5일간 총 보행한 평균 거리는 36.30±50.06m로 나타났다. 2. 사회적 지지가 낮을수록 수술 후 보행시작시간은 지연되었으며(r=-0.331, p=.006), 더 적은 거리를 보행하는 것으로 나타났다(r=0.334, p=.006). 또한 낙상에 대한 두려움이 낮을수록 통계적으로 유의하게 조기보행하는 것을 확인하였다(r=-0.307, p=.011). 3. 지각된 유익성(r=0.416, p=<.001)과 자기효능감(r=0.351, p=.004)이 높고, 지각된 장애성이 낮을수록 유의하게 조기보행 하였으며(r=-0.319, p=.009), 지각된 유익성(r=-0.567, p=<.001)과 자기효능감(r=-0.444, p=<.001)이 클수록 고관절 수술 수 보행 시작 시간이 빨랐던 반면, 지각된 장애성(r=0.389, p=.001)이 클수록 고관절 수술 후 보행 시작 시간이 늦어졌다. 또한 지각된 유익성(r=0.353, p=.003)과 자기효능감(r=0.366, p=.002)이 높을수록 고관절수술 후 더 많은 거리를 보행하는 것으로 나타났다. 결론적으로 고관절 골절이 있는 노인이 수술 후 조기보행을 하는 데에 사회적 지지 및 낙상에 대한 두려움이 유의한 관계가 있었으며, 지각된 유익성과 장애성, 자기효능감 또한 조기보행과 유의한 관계가 있다는 것을 확인하였다. 따라서 고관절 수술 후 조기보행에 영향을 미치는 인지·정서적 요인을 파악하기 위한 후속연구가 필요할 것으로 생각되며, 고관절 수술 후 조기보행을 증진시키기 위한 전략으로 대상자의 인지·정서적 요인을 중재할 것을 제언한다.

Background: The incidence of hip fractures in older adults has been increasing. Long-term bed rest after hip surgery brings various complications to older patients, and increases the social costs. Therefore, it is necessary to identify the relationship of early ambulation with social support and fear of falling in older adults after hip surgery and to increase the patients’ ability to walk. Method: A cross-sectional survey was conducted at a tertiary general hospital in Seoul from February to May 2017. Structured questionnaires based on Pender’s Health Promotion Model and walking reports during 5 days after hip surgery were collected from 67 patients. The data were analyzed with descriptive statistics, a t-test, a one-way ANOVA, a Chi-square test, Fisher’s exact test and Pearson’s correlation. Results: After hip surgery, 41.8% of the patients showed early ambulation (i.e., walking within 48 hours after the surgery). Early ambulation was related with higher social support and a lower fear of falling. In addition, the higher the perceived benefits and the self-efficacy, the earlier the patients walked after surgery. The lower the perceived barriers, the earlier the patients walked after surgery. Conclusion: Early ambulation after hip surgery has a significant relationship with social support and fear of falling; the perceived benefits and self-efficacy; and perceived barriers. As a result, there is a need for a concern in patients’ cognitions and affects, and a strategy to improve early ambulation after hip surgery.
