The present study aims to determine the effect of lower extremity exercise at the non-paralyzed side of patients with stroke using a mirror on gait and balance of patients. Subjects were assigned randomly to a group of lower extremity exercise without using a mirror (n = 10), a group of lower extremity exercise using a mirror (n = 10), and a group of lower extremity muscle strength exercise using a mirror (n = 10). The exercise is added to a physical therapy program conducted at the hospital and subjects conducted their designed lower extremity exercises 30 times a day (5 sets), five days per week for four weeks. The study measured muscle strength, balance ability, and gait ability of subjects before and after the intervention. A paired samples t-test was employed to compare the groups before and after the intervention. The one-way analysis of variance (ANOVA) was used to compare the result between groups. Thus, the following results were obtained. The study result showed that muscle strength had a significant difference within each of the groups before and after the intervention, and Berg balance scale and timed up and go test had a significant difference as well. In the comparison on gait ability, there was a significant difference within two groups using a mirror before and after the intervention, and there was a significant difference in a stride length at the paralyzed side, a step length at the paralyzed side, a step width at the paralyzed side, and single limb support time at the paralyzed side between groups. Conclusively, low extremity exercise using a mirror and low extremity muscle strength exercise using a mirror conducted along with general physiotherapy exercises had a positive effect on functions of lower extremity in patients and the recovery of paralyzed side thereby incurring a significant difference in balance and gait abilities. However, no significant differences were found in balance and gait abilities between group of lower extremity exercise using a mirror and group of lower extremity muscle strength exercise using a mirror.
