Gestational diabetes mellitus is a disease state characterized by carbohydrate intolerance in various levels which started or discovered for the first time in response to the metabolic stress of pregnancy. It is well known that GDM increases the risks for adverse perinatal outcomes, and maternal obesity and excessive weight gain during pregnancy are known for the major risk factors. The present study aims to identify the relationships among prepregnancy BMI, weight gain during pregnancy, and adverse perinatal outcomes of GDM patients. Among 248 patients who were diagnosed as gestational diabetes mellitus at Gangnam CHA Hospital between January 2010 and December 2012, the medical records of 218 women were analysed except the cases of twin prenancy, pregestational diabetes and the transfer of their babies to other hospitals. For Asians, Korean Society for the Study of Obesity defined obesity as more than 25 in BMI which is different from the criteria of NIH (BMI>30). However, we followed NIH criteria in this study. In 2009, Institute of Medicine (IOM) announced the guideline for proper weight gain during pregnancy. However, considering that Korean women have usually shown rather lower weight gain during pregnancy, in this study, we followed Park's classification (Modified IOM Guidelines) which defined inadequete weight gain as less than 5.0 kg in overweight women, and less than 4.0 kg in obese women, but the criteria of excessive weight gain followed IOM Guidelines. For the 218 women with GDM, 70% of the women were normal BMI , 10% under weight, 16% overweight, and only 4% obese ; 11.5% of women showed inadequate weight gain, 33.5% normal weight gain and 55% of them gained more weight than the criteria. BMI, total weight gain, family history of DM, thyroid diseases, and parity were corelated with maternal obesity (P0.05). Fasting plasma insulin, Hb A1c, 100g-oral glucose tolerance test, and blood pressure at 35th week of pregnancy showed significant correlation with BMI (p<0.05). Gestational age (mean : 37.9 weeks) was significantly decreased in the case of higher BMI women (p=0.0006), and it was slightly increased as pregnancy weight gain (p=0.0696). Birth weight (mean : 3,145g) was not significantly related to BMI (p=0.7803), but it increased according to the gain of weight (p=0.0013). Preterm delivery (11%) was none in underweight women, 29% in overweight women, 22% in obese women, and when classified according to total weight gain, the incidences were 12. 16. 7% each, so it showed no significant relation with BMI and weight gain. Large for gestational age fetuses (11%), and macrosomia (6%) were correlated with BMI and total weight gain. Small for gestational age fetuses (7%) was high in the women of inadequate weight gain. Primary Cesarean section (43%) was increased as BMI did, but no correlation with the increase of weight was found. The ratio of pure Cesarean section related to GDM such as induction failure, cephalopelvic disproportion and fetal distress was only 28%. Pregnancy induced hypertension occurred 1% in excessive weight gain group. Adverse perinatal outcomes (maternal outcomes : premature delivery, stillbirth, primary Cesarean section rate, shoulder dystocia, fetal/ neonatal outcomes : macrosomia, fetuess of large for gestational age, small for gestational age, admission to neonatal intensive care unit, neonatal glucose injection, phototherapy, fractures, and congenital anomaly) was predicted by logistic regression analysis. Predictor variables were : maternal age, BMI, total weight gain, mean postprandial blood glucose level at 3rd trimester, fasting plasma insulin, and insulin treatment. Among the predictors, only Hb A1c levels at 3rd trimester were significantly associated. As a result of this study, prevalence of GDM was 2.0%, which was more involved in excessive weight gain during pregnancy than prepregnancy BMI. If even normal BMI women gain excessive weight gain during pregnancy, they showed the higher risks of GDM, and 3rd trimester Hb A1c is significantly corelated with adverse perinatal outcomes. Due to much improved management (treatment stragedy) of GDM women compared to the past, adverse perinatal maternal and neonatal complications seemed to be decreased definitely.
