Patients with upper limb motor nerve conduction velocities (NCVs) under 38-42 m/s are classified as CMT1 type and patients with NCVs greater than 38-42 m/s as CMT2 type. CpG accounts for less than 1% in the human genome and 70-80% of CpG is methylated. Methylation of the CpG island of the promoter located near transcription start site reduces gene expression. Because CMT1A patients have varying symptoms, it is important to develop accurate diagnoses and tailored treatment. In this study, methylation sequencing was performed on DNA from 31 patients and control, and 91 CMT disease markers and 56 symptom markers with significant group differences were selected. Thereafter, neuronal guanine nucleotide exchange factor (NGEF), fibroblast growth factor 22 (FGF22) and fibroblast growth factor receptor 2 (FGFR2), which are thought to affect diseases through gene analysis, were found. In addition, methylation levels were measured by performing pyrosequencing on 13 CpGs in FGFR2 with 97 CMT1A patient samples. As a result, there was a significant difference between patient and control or patient groups classified according to functional disability scale (FDS), CMT neuropathy score (CMTNS) and onset age. There was a significant difference in three CpGs between control and patients. Statistical results were similar on 13 CpG site in the subdivided groups according to the symptoms (FDS, CMTNS, onset age) of the male patients compared with the control and the whole patients. Because only male controls are used in this study, this means that methylation is affected by sex and female controls need to be further analyzed . Compared with other CMT1A severity analysis, there was a significant difference in the groups of male patients classified as early onset age (≤10) and late onset age (>10) according to the onset age. This suggests that methylation of FGFR2 is associated with the onset age of CMT1A. This study showed that there were significant differences among the 140 markers and CpG island in FGFR2. Epigenetic methylation studies on CMT1A patients may be applicable to tailored therapy for patients.
