Results : 근감소를 가질수록 연령이 높았으며, 경제활동에 종사하는 비율이 유의적으로 낮았다. 또한 근감소가 있는 군의 격렬한 신체활동, 중증도 신체활동, 걷기 등의 신체활동 실천이 유의적으로 낮은 것으로 나타났다. 신체활동량을 표준화한 대사당량(Metabolic Equivalent Task)의 수준이 높아질수록 근감소성 비만에 대한 위험이 감소되었다. 대상자들의 영양소 섭취량을 분석하였을 때, 근감소가 있는 군은 비근감소군에 비해 유의적으로 에너지 섭취가 적었고, 탄수화물 섭취 비율이 낮았으며 단백질 섭취 비율이 높았다. 에너지 필요추정량을 충족하지 못하는 군은 충족군에 비해 근감소성 비만에 대한 위험이 1.24배 높았으며, 리보플라빈과 비타민 C의 권장섭취량을 충족하지 못하는 군은 충족군에 비해 근감소성 비만에 대한 위험이 각각 1.36배, 1.3배 높아지는 것으로 나타났다. 혈액 항목에서는 근감소가 있을수록 높은 혈중 중성지방 농도와 낮은 HDL-콜레스테롤 농도를 가지고 있었으며, 높은 공복혈당과 인슐린저항성 수준을 가지고 있었다. 근감소군이 비근감소군에 비해 혈중 비타민 D 농도는 유의적으로 낮게 나타난 반면 부갑상선호르몬 농도는 유의적으로 높게 나타났다. 대상자들이 현재 가지고 있는 질병 유병률을 보았을 때, 근감소가 있을수록 당뇨병, 고혈압, 이상지질혈증의 유병률이 높았다. 근감소증 및 비만 여부에 따른 대사증후군 위험도 분석에서는 정상군을 기준으로 대사증후군에 대한 위험이 근감소군은 1.99배, 비만군은 10.89배, 근감소성 비만군은 10.99배 높은 것으로 나타났다.
Conclusions : 본 연구의 분석 결과, 노화로 인한 근감소증은 대사증후군 및 당뇨병의 위험을 높이는 것으로 나타났고, 특히 근감소증이 단독으로 존재할 때 보다 근감소증과 비만이 함께 동반될 때 대사증후군과 같은 만성질환에 대한 위험에 더욱 노출되었다. 그러나 활동적인 신체활동과 충분한 열량섭취와 함께 항산화 영양소가 풍부한 식사는 근감소증 및 근감소성 비만을 예방하는데 도움을 줄 수 있다. 따라서 노인은 각자의 운동 능력과 활동 수준을 고려하여 신체활동 수준을 늘리고, 충분한 열량 섭취와 더불어 비타민 C와 리보플라빈 등의 항산화 영양소가 풍부한 식사를 섭취한다면 건강한 근육상태를 유지할 수 있을 것이다.

Such changes in body composition are reported to have an adverse effect on indicator related to health of the elderly. The purpose of this study is to provide basic data for prevention and management of sarcopenia and sarcopenic obesity by analyzing of factors related with sarcopenia and sarcopenic obesity using data of 2008-2011 National Health and Nutrition Survey.
Subjects and Methods : The present study was a cross-sectional study using the National Health and Nutrition Survey from the fourth to fifth period conducted from 2008 to 2011. Among the 6,370 elderly people aged 65 or older who participated in this survey, 3,367 subjects were included in the final analysis after excluding 3,003 subjects who had missing data. Subjects were divided into the normal group, the sarcopenia group, the obesity group, and the sarcopenic obesity group depeding on existence of sarcopenia and/or obesity. The general characteristics, physical measurement information, physical activity level, nutrient intake status, disease prevalence, odds ratio of metabolic syndrome were analyzed.
Results : The group with sarcopenia had higher age, and the rate of occupation in economic activity was significantly lower. In addition, Physical activities such as vigorous physical activity, moderate physical activity, and walking activity were significantly lower in the group with sarcopenia. As the level of Metabolic Equivalent Task, which is the standard of physical activity, was increased, the risk for sarcopenic obeisty was decreased. When the subjects' nutrient intake was analyzed, The group with muscle decline had significantly lower energy and carbohydrate intake, and higher protein intake than non muscle decline group. Those who did not meet the energy requirement estimates had a 1.24-fold higher risk for sarcopenic obesity than those who did not. furthermore, Those who did not meet the recommended intake of riboflavin and vitamin C had a 1.36 times and 1.3 times higher risk of Sarcopenic obesity, Respectively. In section of blood profile, There was a high serum triglyceride level and low HDL-cholesterol concentration with muscle decline, and high fasting blood glucose and insulin resistance levels. Serum vitamin D levels were significantly lower in the group with muscle decline compared to the non muslce decline group, but the parathyroid hormone concentration was significantly higher. The prevalence of diabetes mellitus, hypertension, and dyslipidemia was found to be higher in group with sarcopenia when we analyzed prevalence of the chronic disease that the subjects currently have. Based on the normal group, The risk of metabolic syndrome was 1.99 times in the sarcopenic group, 10.89 times in the obese group, 10.99 times in the sarcoepnic obesity.
Conclusions : The results of this study showed that sarcopenia related to aging increases the risk of metabolic syndrome and diabetes. Especially when sarcopenia is accompanied with obesity, It was more exposed to chronic diseases and adverse effect on indicators related to health. However, Active physical activity and adequate calorie intake, as well as antioxidant-rich meals, can help prevent sarcopenia and sarcopenic obesity. Therefore, The elderly will be able to maintain their healthy muscular state by increasing physical activity level considering their athletic ability and activity level, and eating a sufficient amount of caloric intake and a diet rich in antioxidant nutrients such as vitamin C and riboflavin.
