본 연구는 성인 모야모야 환자의 질병 지식, 불확실성 및 삶의 질 정도를 확인하고, 상관관계를 파악하여 삶의 질에 미치는 영향을 분석하고자 시도된 서술적 상관관계 연구이다. 자료 수집은 2017년 3월 27일부터 2017년 11월 24일까지 서울소재 Y대학 병원 입원 및 외래를 방문하는 성인 모야모야병 환자 90명을 대상으로 직접 면담을 통한 설문지법으로 이루어졌다. 연구 도구는 인지장애가 없는 대상자 선별을 위한 K-MMSE, 모야모야 환자의 질병 지식을 측정하기 위해 연구자 가 개발하고 전문가 집단에게 타당도를 검증받은 질병 지식 측정 도구, Mishel(1988)이 개발한 불확실성 측정 도구를 국문으로 번역한 도구(Chung, 2005), Ware과 Sherbourne(1992)이 개발한 삶의 질 측정도구(SF-36)를 이상 열(2001)이 한국판으로 표준화한 도구를 사용하였다. 수집된 자료는 SPSS 24.0프로그램을 이용하여 기술통계, t-test, ANOVA, Pearson’s correlation, Multiple liner regression을 실시하였으며 연구의 결과는 다음과 같다. 1. 성인 모야모야 환자의 일반적 특성은 여자가 61명(67.8%)이었고, 평균 나 이는 41.12(±9.98)세였다. 결혼 상태는 기혼이 69명(76.7%)이었고, 직업 은 회사원이 44명(48.9%)으로 가장 많았다. 교육 정도는 대학교 졸업 이 상이 55명(61.1%)으로 가장 많았다. 2. 성인 모야모야 환자의 임상적 특성은 진단을 받게 된 경로에서 증상이 있 어서 병원을 방문하였다가 진단을 받은 경우가 74명(82.2%)으로 가장 많았고, 평균 진단 기간은 3.54(±3.56)년이었다. 본인이 느끼는 질병 심각 도는 ‘심각하다’고 느끼는 사람이 49명(54.4%)로 가장 많았고, 질병에 대 한 정보를 얻는 경로는 인터넷을 통해서 얻는 방법이 74명(82.2%)이었다. 일과성 허혈성 발작을 경험한 환자는 78명(86.7%)이었으며, 뇌출혈은 13 명(14.4%), 뇌경색은 17명(18.9%), 경련은 6명(6.7%)의 환자들이 경험하였다. 3. 성인 모야모야 환자의 질병 지식은 총 12점 만점에 평균 9.65(±1.84)점 으로 대상자의 나이(F=3.66, p =.030), 교육 정도(t=-2.02, p =.046), 진단 경로(F=8.85, p = . 001), 일과성 허혈성 발작 유무(t=-2.21, p =.030), 수술 유무(t=-2.82, p =.006)에 따라 통계적으로 유의한 차이를 나타냈다. 4. 성인 모야모야 환자의 불확실성은 총점 평균 93.11(±15.19)점으로, 하위 항목에서 불예측성이 15.38±3.17(평균 평점 3.07±0.63)점으로 가장 높 았고, 이는 음주여부(F=3.50, p= .034), 진단 기간(F=3.65, p= .030)에 따라 유의한 차이를 나타냈다. 5. 성인 모야모야 환자의 삶의 질은 백점 만점 환산 시, 신체적 삶의 질이 65.31(±16.15)점, 정신적 삶의 질이 60.11(±19.13)점으로 나타났고, 세 부 항목으로, 신체적 삶의 질 중에서는 신체적 기능이 79.61(±18.46)점, 정신적 삶의 질 중에서는 사회적 기능이 68.33(±20.88)점으로 가장 높게 나타났다. 신체적 삶의 질은 성별(t=3.51, P =.001), 직업(F=5.76, p =.001), 진단 경로(F=4.72, p =.011), 본인이 느끼는 질병 심각도(F=7.90, p =.001), 뇌경색 유무(t=2.04, p= .044), 경련 유무(t=2.41, p =.034)가 유 의한 것으로, 정신적 삶의 질은 성별(t=2.63, p =.010), 결혼 유무(t=-2.29, p =.024), 본인이 느끼는 질병 심각도(F=5.42, p =.006), 뇌경색 유무(t=2.10, p =.038)가 유의한 것으로 나타났다. 5. 성인 모야모야 환자의 질병 지식, 불확실성 및 삶의 질의 상관관계에서 는 질병 지식이 증가할수록 불확실성이 유의하게 낮아지고(r=-.325, p =.002)를, 불확실성이 낮을수록 신체적 삶의 질(r=-3.51, p =.001), 정 신적 삶의 질(r=-.346, p= .001)이 유의하게 높아진다고 나타났다. 6. 성인 모야모야 환자의 삶의 질에 영향을 미치는 요인으로는 신체적 삶의 질에서 직업, 진단 경로, 본인이 느끼는 질병 심각도, 경련 유무, 불확실 성이었고, 모형 설명력은 48.9%로 나타났으며, 정신적 삶의 질에서는 결 혼 유무, 직업, 진단 경로, 본인이 느끼는 질병 심각도 및 불확실성이 유 의한 영향 요인이었고, 모형 설명력은 31.4%였다.
본 연구는 성인 모야모야 환자를 대상으로 질병 지식, 불확실성 및 삶의 질 의 관련성을 파악하고, 이에 미치는 영향 요인을 규명하여 삶의 질 향상을 위 한 효과적인 간호 중재의 기초 자료를 마련하고자 시도된 첫 연구로서, 질병 지식과 불확실성, 불확실성과 삶의 질이 유의한 상관관계에 있음을 확인하였 다. 이를 바탕으로 성인 모야모야 환자의 질병 특성 및 개인적 특성에 맞는 구체적이고 체계적인 교육 제공을 통해 질병 지식의 증대 및 불확실성을 감소 시키고 궁극적으로 삶의 질을 증진시킬 수 있는 간호 중재가 필요하며 이러한 연구가 지속적으로 이루어져야 할 것으로 사료된다.

Background Moyamoya disease is a chronic, progressive cerebrovascular disease with an unconfirmed cause. Patients who develop the disease require regular check-ups throughout their lives and require ongoing management. However, there is a lack of research on adult patients who suffer from moyamoya. Therefore, this study aims to examine the relationships among knowledge, uncertainty, and quality of life in patients with adult moyamoya disease. Furthermore, the factors associate with the quality of life of adult patients with moyamoya were investigated. Methods The design of the study is a descriptive correlation study. Data were collected from a convenient sample of 90 adult patients with moyamoya disease who were hospitalized or visited the outpatient clinic at one university hospital located in Seoul between March 27, and November 24, 2017. A knowledge assessment tool composed of 12 items was developed and validity of the tool was verified by expert groups. In addition, uncertainty measurement tool with 33 items, and quality of life measurement tool with 36 items were also completed by each patient. The general and clinical characteristics were collected using surveys and electronic medical records. The data were analyzed with descriptive statistics, t-test, ANOVA, Pearson’s correlation, and multiple liner regression using SPSS 24.0. Results The mean of disease-related knowledge of adult patients with moyamoya was 9.65(±1.84) points out of 12, with significant differences in scores by age, level of education, diagnosis route, presence of transient ischemic attacks and surgeries. The mean of uncertainty was 93.11(±15.19); among the sub-categories, unexpectedness was highest, 15.38(±3.17). The score of uncertainty were significantly different by alcohol drinking and disease duration. The physical quality of life was 65.31(±16.15) and mental quality of life was 60.11(±19.13) for out of 100; Among the sub-categories, physical functions of physical quality of life was 79.61 and social functions of mental quality of life was highest at 68.33. The physical quality of life was significantly differed by gender(t=3.51, p=.001), occupation(F=5.76, p=.001), diagnosis route(F=4.72, p=.011), severity of the disease(F=7.90, p=.001), infarction(t=2.04, p=.044), as well as the presence of seizures(t=2.41, p=.034). The mental quality of life was significantly differed by gender(t=2.63, p=.010), marital status(t=-2.29, p=.024), severity of the disease(F=5.42, p=.006) and infarction(t=2.10, p=.038). The significant correlation showed lower uncertainty with higher disease-related knowledge(r=-.325, p=.002) and lower uncertainty with higher physical(r=-3.51, p=.001) and mental(r=-.346, p=.001) quality of life. Lastly, statistically significant factors associated with the physical quality of life included occupation, diagnosis route, severity of the disease, seizures and uncertainty, and the regression model explained physical quality of life by 48.9%. When the mental health quality of life was assessed, the associated factors were found to be marital status, occupation, diagnosis route, severity of disease and uncertainty, and the regression model explained mental quality of life by 31.4%. Conclusion The findings of this study indicated that it is necessary to develop nursing intervention to increase a patient’s knowledge of their disease as reducing uncertainty can ultimately help improve quality of life. In addition, comprehensive education that is customized for patients is needed for optimal patient management.
